摘要
目的探讨循环肿瘤细胞(CTC)检测对胃癌新辅助化疗(NAC)疗效的评估价值。方法选取2016年10月—2018年10月我院收治的90例胃癌患者进行前瞻性研究。利用SE-iFISH技术检测胃癌患者NAC前后CTCs,分析NAC前胃癌患者CTCs与临床病理特征的关系。以病理学Miller-Payne标准作为评估疗效的金标准,评估CTCs对NAC疗效评价的准确性。结果NAC前CTCs阳性与肿瘤浸润深度、淋巴结转移状态显著相关(P<0.05);胃癌患者NAC前CTCs阳性率为86.67%(78/90),NAC后CTCs阳性率为48.89%(44/90),差异具有统计学意义(P<0.05);CTCs检测评估NAC疗效的敏感度、特异度、准确性分别为88.24%、68.18%、83.33%,与金标准评估疗效中度一致(Kappa=0.556,P<0.001)。结论CTCs检测对评价胃癌患者NAC疗效具有一定的临床价值。
Objective To study the value of circulating tumor cells(CTCs)detection in the evaluation of neoadjuvant chemotherapy(NAC)for gastric cancer.Methods 90 patients with gastric cancer admitted to our hospital from October 2016 to October 2018 were collected for prospective study.The CTCs of patients with gastric cancer were detected by SE-iFISH technique before and after NAC,and the relationship between CTCs and clinical pathological features were analyzed before NAC.According to the pathological evaluation standard of Miller-Payne as a gold standard for curative effect evaluation,the accuracy of CTCs in the evaluation of the effect of NAC was analyzed.Results The positive rate of pre-NAC CTCs was significantly correlated with the tumor infiltration depth and lymph node metastasis(P<0.05).The positive rate of pre-NAC CTCs for patients with gastric cancer was 86.67%(78/90),and the positive rate of pro-NAC CTCs was 48.89%(44/90).There was statistically significant difference between the positive rate of pre-NAC CTCs and pro-NAC CTCs(P<0.05).The sensitivity,specificity and accuracy of CTCs detection in the evaluation of NAC effect were 88.24%,68.18%and 83.33%,respectively,which was moderately consistent with the gold standard assessed efficacy(Kappa=0.556,P<0.001).Conclusion CTCs detection has certain clinical value in the evaluation of NAC effect for patients with gastric cancer.
