摘要
目的综合评价不同频率分散式体外冲击波治疗带状疱疹后神经痛的疗效。方法 2018年1月~2019年1月共选择纳入标准的下肢股神经带状疱疹后神经痛患者160例,随机分为对照组、5Hz治疗组、10Hz治疗组和15Hz治疗组4组,每组40例,治疗期间所有患者均给予普瑞巴林胶囊口服治疗,5、10和15Hz治疗组分别给予5、10和15Hz分散式体外冲击波(1次/周,4次)治疗,比较4组治疗前后疼痛程度、抑郁情况和下肢股神经运动传导速度(MNCV)变化特点,并记录患者不良反应。结果疼痛强度评分比较:各组治疗后相较于治疗前均明显降低(P<0.05),此外治疗后与对照组组相比,10 Hz治疗组明显降低(P<0.05)。焦虑症状评分:各组治疗后相较于治疗前均明显降低(P<0.05),此外治疗后与对照组组相比,10和15Hz组明显降低(P<0.05)。肌电图检测:治疗前后、各组间比较均无统计学意义(P>0.05);不良反应:15Hz组出现不良反应5例,局部淤斑4例、肿胀1例,均自行恢复,其发生率高于对照组、5和10 Hz组(P<0.05)。结论体外冲击波治疗带状疱疹后神经痛效果显著,但不同的频率有一定的差异,其中以10Hz治疗效果较好,同时不会引起神经损害和副作用。
Objective To compare the curative effect of differences frequency radial extracorporeal shock wave in post herpes zoster neuralgia. Methods 160 patients of lower limb femoral nerve neuralgia post herpes zoster were randomly divided into control group, 5 Hz treatment group, 10 Hz treatment group and 15 Hz treatment group. All patients were routinely given pregabalin capsules during treatment period, and then, 5, 10, and 15 Hz treatment groups were given 5, 10, and 15 Hz decentralized extracorporeal shock waves(1 time/week for 4 times), after make a comparison for pain degree, depression, and changes in the lower extremity femoral nerve conduction velocity(MNCV) before and after treatment, in addition documented adverse events. Result The comparison of pain intensity scores of treatment group was significantly lower than that before treatment(P<0.05). In addition, compared with the control group, the 5, 10, and 15 Hz treatment groups were significantly lower(P<0.05). The anxiety symptom scores. The treatment group were significantly lower than them before treatment(P<0.05). In addition, compared with the control group, the 10 and 15 Hz groups were significantly lower(P<0.05). There was no significant difference of electromyography between the two groups before and after treatment(P>0.05). 5 cases of adverse reactions occurred in the 15 Hz group, 4 cases of local ecchymoses and 1 case of swelling, all recovered spontaneously. The rate was higher than that them in the control group, 5 and 10 Hz(P<0.05). Conclusion Extracorporeal shock wave treat post-herpetic neuralgia is effective, but there are certain differences in frequency among them. 10 Hz is better, and it does not cause nerve damage and side effects.
