摘要
目的比较羟考酮与舒芬太尼术后镇痛对结肠癌患者免疫功能的影响。方法选取医院2016年9月至2018年6月收治拟行腹腔镜结肠癌根治术的结肠癌患者118例,按随机数字表法分为观察组和对照组,各59例。两组均常规在腹腔镜辅助下实施结肠癌根治术,术后观察组给予羟考酮镇痛,对照组给予舒芬太尼镇痛。结果两组患者麻醉镇痛指标相比,差异不显著(P> 0. 05)。组内比较,观察组患者术后8 h和术后16 h的CD4+、术后8 h的CD8+和自然杀伤细胞(NK)水平均明显低于术毕,且术后16 h的CD8+和NK细胞水平均明显高于术后8 h(P <0. 05);对照组患者术后8 h和术后16 h的CD4+,CD8+,NK细胞水平均明显低于术毕,且术后16 h的CD4+水平明显低于术后8 h,NK细胞水平明显高于术后8 h(P <0. 05)。组间比较,两组患者术毕的免疫功能指标相比,差异不显著(P> 0. 05);观察组患者术后8 h和术后16 h的CD4+,CD8+,NK细胞水平均明显高于对照组(P <0. 05);观察组患者不良反应总发生率为5. 08%,明显低于对照组的16. 95%(P <0. 05)。结论羟考酮与舒芬太尼对结肠癌患者的镇痛效果均较好,但羟考酮对机体免疫功能的影响相对更小,且安全性相对较高。
Objective To compare the effect of postoperative analgesia with oxycodone or sufentanil on the immune function of patients with colon cancer. Methods Totally 118 patients with colon cancer underwent laparoscopic radical resection of colon cancer admitted to our hospital from September 2016 to June 2018 were selected and divided into the observation group and the control group according to the random number table method,59 cases in each group. The patients in the two groups underwent radical resection of colon cancer assisted by laparoscopy. After operation,the patients in the observation group were given oxycodone for analgesia,while the patients in the control group were given sufentanil for analgesia. Results There was no significant difference in analgesic index between the two groups( P > 0. 05). The levels of CD4+at 8 h and 16 h after operation,CD8+and natural killer( NK) cells at 8 h after operation in the observation group were significantly lower than those at the end of operation,and the levels of CD8+and NK cells at 16 h after operation were significantly higher than those at 8 h after operation( P < 0. 05). The levels of CD4+at 8 h and 16 h after operation,CD8+and NK cells at 8 h after operation in the control group were significantly lower than those at the end of operation,and the level of CD4+at 16 h after operation was significantly lower than that at 8 h after operation,while the level of NK cells at 16 h after operation was significantly higher than at 8 h after operation( P < 0. 05). There was no significant difference in immune function indexes between the two groups at the end of operation( P > 0. 05). The levels of CD4+,CD8+and NK cells in the observation group were significantly higher than those in the control group at 8 h and 16 h after operation( P < 0. 05). The total incidence of adverse reactions in the observation group was 5. 08%,which was significantly lower than 16. 95% in the control group( P < 0. 05). Conclusion Oxycodone and sufentanil have good analgesic effect on patients with
