摘要
目的探讨口腔恶性肿瘤患者就医及治疗过程中的生理、心理体验及其影响因素,为提出改善患者生存质量的措施及后续研究提供依据。方法采用质性研究方法,就身体状况、社会家庭生活、情感状况、头颈癌患者术后相关问题、延续护理需求及就医体验等方面,以半结构式访谈提纲对15例口腔恶性肿瘤术后患者进行深入访谈,运用Nvivo11质性研究辅助分析软件及Colaizzi现象分析法对访谈资料进行整理和分析。结果经阅读、归纳,将访谈资料分为“入院前阶段”“住院阶段”及“出院后阶段”3个部分,共提炼出8个主题。即入院前阶段:就医便利性差、医疗信息匮乏、家庭支持不连续;住院阶段:负面情绪疏导不畅、机体功能障碍;出院后阶段:延续医疗服务不完善,出院指导缺乏长期方案、患者依从性差。结论积极探索符合口腔恶性肿瘤疾病特点的延续护理系统,是今后改善患者就医体验,提高患者生存质量的重要途径。
Objective To investigate the physiological and psychological experiences during the treatment process in the patients with oral malignant tumor and its influencing factors in order to provide the evidence for formulating the measures improving the quality of life and follow up study. Methods Adopting the qualitative research methods,in the aspects of the physical condition,social and family life,emotional status,postoperative related problems in the patients with head and neck cancer,continuous nursing need and medical experience,15 postoperative patients with oral malignant tumor were conducted to the deep interviews with semi-structured interview outline.Then the interview data were sorted and analyzed by the Nvivo 11 qualitative auxiliary analysis software and Colaizzi phenomenon analysis method. Results After reading and summarizing,the interview data were divided into three parts:"pre-admission stage","hospitalization stage" and "post-discharge stage",and a total of 8 topics were extracted,i.e.,in the pre-admission stage,the convenience of medical service was poor,the medical information was scarce and the family support was discontinuous.In the hospitalization stage,the negative psychological counseling was not free,the organism function obstacle;in the post-discharge stage,the continued medical service was not good enough the discharge guidance lacked a long-term plan and patient′s compliance was poor. Conclusion Actively exploring the continous nursing system conforming to the characteristics of oral malignant tumor is an important way to improve the patients′ medical experience and increase their quality of life in the future.
