摘要
目的:探讨口腔颌面部恶性肿瘤(OMMs)患者术后生活质量现状及其影响因素。方法:以方便抽样方式选择2020年1月-2021年11月在中山大学附属第一医院行手术治疗的98例OMMs患者,分析OMMs患者术后生活质量现状及其影响因素。结果:OMMs患者术后生存质量评分为(63.59±4.56)分,其中生活质量好(>76分)42例(42.86%),生活质量差(≤76分)56例(57.14%);单因素分析显示,是否有下颌骨节段切除、吞咽功能障碍、不同社会支持的OMMs患者术后生活质量评分比较,差异均有统计学意义(P<0.05)。不同性别、年龄、文化程度、职业、医疗费用支付方式、感染、婚姻状况、颈淋巴清扫术、皮瓣移植修复、伤口疼痛、家庭人均月收入OMMs患者的生活质量评分比较,差异均无统计学意义(P>0.05)。多因素分析显示,下颌骨节段切除[OR=4.500,95%CI(1.896,10.680)]、社会支持<45分[OR=2.727,95%CI(1.159,6.419)]、吞咽功能障碍[OR=13.444,95%CI(5.071,35.644)]是OMMs患者术后生活质量差的影响因素(P<0.05)。结论:OMMs患者术后生活质量较差,其受下颌骨节段切除、社会支持、吞咽功能障碍影响。
Objective:To investigate the status and influencing factors of postoperative quality of life in patients with oral and maxillofacial malignancies(OMMs).Method:A total of 98 patients with OMMs who underwent surgical treatment in the First Affiliated Hospital of Sun Yat-sen University from January 2020 to November 2021 were selected by convenient sampling,and the postoperative quality of life of patients with OMMs and its influencing factors were analyzed.Result:The postoperative quality of life scores of patients with OMMs was(63.59±4.56)points,including 42 cases(42.86%)with good quality of life(>76 points)and 56 cases(57.14%)with poor quality of life(≤76 points).Univariate analysis showed that there were statistically significant differences in postoperative quality of life scores of OMMs patients with or without mandibular segmental resection,swallowing dysfunction and different social support(P<0.05).There were no significant differences in the quality of life scores of OMMs patients with different gender,age,education level,occupation,payment method of medical expenses,infection,marital status,neck dissection,flap transplantation and repair,wound pain and monthly income(P>0.05).Multivariate analysis showed that mandibular segmental resection[OR=4.500,95%CI(1.896,10.680)],social support<45 points[OR=2.727,95%CI(1.159,6.419)],swallowing dysfunction[OR=13.444,95%CI(5.071,35.644)]were the influencing factors of poor postoperative quality of life in patients with OMMs(P<0.05).Conclusion:The postoperative quality of life of patients with OMMs is poor,which is affected by mandibular segmentectomy,social support and swallowing dysfunction.
