摘要
目的系统评价常见活血化瘀类中药注射液联合甲钴胺治疗糖尿病周围神经病变(diabeticperipheralneuropathy,DPN)的临床效果。方法 查询PubMed、知网、万方和维普等数据库,并人工检索相关杂志、会议报道。收集活血化瘀类中药注射液联合甲钴胺治疗DPN患者随机对照试验(RCTs),检索时间为建库至2017年12月,以ADDIS1.16.8软件进行网状Meta分析与最佳概率排序。结果 共纳入52项研究,4352例患者。(1)总有效率:10种中药注射液联合甲钴胺均比甲钴胺单用的总有效率高(P<0.05)。(2)正中神经运动神经传导速度(MNCV):10种中药注射液联合甲钴胺均比甲钴胺单用提高正中神经MNCV效果好(P<0.05)。(3)腓总神经MNCV:10种中药注射液联合甲钴胺均比甲钴胺单用提高腓总神经MNCV效果好(P<0.05)。(4)正中神经感觉神经传导速度(SNCV):除葛根素、脉络宁外的8种中药注射液联合甲钴胺比甲钴胺单用提高正中神经SNCV效果好(P<0.05)。(5)腓总神经SNCV:除脉络宁外的9种中药注射液联合甲钴胺比甲钴胺单用提高腓总神经SNCV效果好(P<0.05)。结论 活血化瘀类中药注射液联合甲钴胺提高总有效率和正中神经、腓总神经MNCV,均以复方丹参效果最优;提高正中神经、腓总神经SNCV优先选择血栓通与疏血通。
Objective To systematically evaluate the clinical efficacy of common chinese medicine injection for promoting blood circulation by removing blood stasis with methycobal in the treatment of diabetic peripheral neuropathy. Methods We searched the PubMed,CNKI,Wanfang,VIP database,and manual retrieved journals and conferences,collecting randomized controlled trial(RCT)of chinese medicine injection for promoting blood circulationto dissipate blood stasis with methycobal in the treatment of diabetic peripheral neuropathy,up to in November 2017 since database establishment.The data were Meta-analyzed by ADDIS 1.16.8 software. Results Fifty-two researches and 4 352 patients were included in the study.The network Meta-analysis and the best sorting analysis showed that,(1)The total efficiency:10 kinds of chinese medicine injection combined with methycobal could improve the total efficiency compared methycobal alone ( P< 0.05).(2)The MNCV of median nerve:10 kinds of chinese medicine injection combined with methycobal can improve the MNCV of median nerve compared methycobal alone( P< 0.05).(3)The MNCV of common peroneal nerve:10 kinds of chinese medicine injection combined with methycobal can improve the MNCV of common peroneal nerve compared methycobal alone( P< 0.05).(4)The SNCV of median nerve:8 kinds of chinese medicine injection combined with methycobal can improve the SNCV of median nerve except Gegensu、Mailuoning compared methycobal alone ( P< 0.05).(5)The SNCV of common peroneal nerve:9 kinds of chinese medicine injection combined with methycobal can improve the SNCV of common peroneal nerve except Mailuoning compared methycobal alone ( P< 0.05). Conclusion Chinese medicine injection for promoting blood circulation to dissipate blood stasis combined with methycobal could improve the total efficiency and the MNCV of median nerve,common peroneal nerve,and the effect of compound danshen was best,which firstly chose firstly choose Xueshuantong and Shuxuetong in improving the SNCV of median nerve,common peroneal nerve.
