外固定器预防Pilon骨折后骨筋膜室综合征 被引量:2

摘要 目的探究高能量闭合性Pilon骨折早期采用外固定器对于预防骨筋膜室综合征的有效性。方法回顾性分析海城市正骨医院2015年10月-2017年9月收治的单侧高能量闭合性Pilon骨折的病人60例,伤后时间均为6 h以内,分为观察组和对照组,各30例,对照组采取跟骨牵引,观察组采用外固定器固定,观察2组治疗后24 h、3 d、7 d的疼痛v AS评分及伤肢肿胀进展情况;对比2组III度肿胀发生率及骨筋膜室综合征急诊切开减张发生率。结果观察组治疗后24 h、3 d、7 d疼痛v AS评分显著低于同期对照组,差异有统计学意义(P<0.05);观察组肿胀程度及骨筋膜室综合征急诊切开减张发生率显著低于对照组,差异有统计学意义(P<0.05)。结论Pilon骨折后早期行外固定器可以减少骨筋膜室综合征的发生几率,疗效显著,值得临床推广。 Objective To investigate the effectiveness of external fixators in the prevention of compartment syndrome in high-energy closed Pilon fractures. Methods A retrospective analysis of 60 patients with unilateral high-energy closed Pilon fractures from October 2015 to September 2017 in Haicheng Bone-Setting Hosptial was performed. The time after injury was within 6 hours. They were randomly divided into observation group and control group with 30 cases in each case. The control group was treated with calcaneal traction, and the observation group was fixed with external fixator. The pain VAS score and the progress of the swollen limb swelling were observed at 24 h, 3 d and 7 d after treatment. The incidence of grade III swelling was compared between the two groups. The incidence of osteocardiosis syndrome in the emergency incision and reduction. Results The vAS scores of the observation group were significantly lower than the control group at 24 hours, 3 days, and 7 days after treatment(P < 0.05). The degree of swelling in the observation group and the emergency incision and reduction of the compartment syndrome. The incidence was significantly lower than that of the control group, and the difference was statistically significant(P < 0.05). Conclusion Early fixation of Pilon fractures can reduce the incidence of compartment syndrome, and the curative effect is significant, which is worthy of clinical promotion.
