摘要
目的探讨清肺祛瘀汤治疗小儿呼吸道合胞病毒(RSV)性肺炎的临床疗效及其对患儿炎症因子的影响。方法选取兴平市人民医院2016年1月至2018年9月收治的96例RSV肺炎患儿作为研究对象,采用随机数表法将其随机分为对照组和观察组,每组48例。对照组患儿给予利巴韦林抗病毒、止咳、吸痰、平喘、退热、调节电解质平衡等常规治疗,观察组在对照组治疗基础上加服清肺祛瘀汤。两组均连续治疗10d。比较两组患儿的治疗效果及治疗前后C反应蛋白(CRP)、肿瘤坏死因子-α(TNF-α)、白细胞介素-8(IL-8)等炎症因子变化。结果观察组和对照组患儿的发热消退时间[(2.18±0.61)dvs(3.69±1.02)d]、止咳时间[(4.64±0.68)dvs(6.58±0.87)d]、平喘时间[(4.35±0.41)dvs(5.71±0.44)d]、痰鸣音消失时间[(4.44±1.07)dvs(6.48±1.68)d]、肺部啰音消失时间[(5.12±0.63)dvs(6.57±0.80)d]、X线片吸收时间[(5.91±0.49)dvs(7.48±0.60)d]及住院天数[(6.24±0.56)dvs(7.97±0.72)d]比较,观察组均明显快于对照组,差异均有统计学意义(P<0.05);观察组和对照组患儿治疗后的血清CRP[(4.18±1.12)mg/Lvs(6.23±1.57)mg/L]、TNF-α[(10.81±2.75)ng/Lvs(18.52±7.46)ng/L]和IL-8[(4.08±0.34)ng/Lvs(4.73±0.49)ng/L]水平比较,观察组明显低于对照组,差异均有统计学意义(P<0.05);观察组患儿的愈显率为77.08%,明显高于对照组的54.17%,差异有统计学意义(P<0.05);观察组和对照组不良反应发生率分别为6.25%和4.17%,差异无统计学意义(P>0.05)。结论在常规治疗基础上应用清肺祛瘀汤可提高小儿RSV肺炎的治疗效果,促进患儿临床症状改善,缓解炎症反应,且安全性良好。
Objective To investigate the clinical efficacy of Qingfei Quyu Decoction in the treatment of infantile respiratory syncytial virus (RSV) pneumonia and its effect on inflammatory factors. Methods Ninety-six children with RSV pneumonia admitted to the People's Hospital of Xingping City from January 2016 to September 2018 were randomly divided into control group and observation group, with 48 children in each group. The children in the control group received routine treatment of ribavirin antivirus, cough relief, sputum aspiration, asthma relief, fever abatement and electrolyte balance regulation, while those in the observation group were given Qingfei Quyu Decoction on the basis of the control group. The therapeutic effect and the changes of inflammatory factors, including of C-reactive protein (CRP), tumor necrosis factor-α(TNF-α), interleukin-8 (IL-8), in the two groups were evaluated and compared. Results The fever subsidence time, cough relief time, asthma relief time, sputum sound disappearance time, lung rale disappearance time, X-ray absorption time, and hospitalization days in the observation group were significantly lower than those in the control group (P<0.05):(2.18±0.61) d vs (3.69±1.02) d,(4.64±0.68) d vs (6.58±0.87) d,(4.35±0.41) d vs (5.71±0.44) d,(4.44±1.07) d vs (6.48±1.68) d,(5.12±0.63) d vs (6.57±0.80) d,(5.91±0.49) d vs (7.48±0.60) d,(6.24±0.56) d vs (7.97± 0.72) d. After treatment, the levels of CRP, TNF-α, and IL-8 in the observation group were significantly lower than those in the control group (P<0.05):(4.18±1.12) mg/L vs (6.23±1.57) mg/L,(10.81±2.75) ng/L vs (18.52±7.46) ng/L,(4.08±0.34) ng/L vs (4.73±0.49) ng/L. The markedly effective rate of the observation group was significantly higher than that of the control group (77.08% vs 54.17%, P<0.05), and there were no serious adverse reactions (6.25% vs 4.17%, P> 0.05). Conclusion On the basis of routine treatment, the application of Qingfei Quyu Decoction can improve the therapeutic effect of RSV pneumonia in children,
