摘要 目的探讨血管内支架成形术(PTAS)治疗缺血性脑血管病对患者认知功能、脑血流参数及神经功能的影响。方法选取我院实施PTAS治疗的颈内动脉重度狭窄且伴有症状的患者49例作为研究对象,观察治疗前后患者大脑中动脉(MCA)的收缩期峰值血流速度(PSV)、搏动指数(PI)、脑血管储备能力(CVR)、动脉狭窄段直径,简易精神状态量表(MMSE)评分、蒙特利尔认知功能量表(MoCA)评分及美国国立卫生研究院卒中量表评分(NIHSS)。结果术后3d复查,49例患者的MCV平均PSV值、PI值、CVR值较术前提高(P <0. 05),患者狭窄段颈动脉直径较手术前增大(P <0. 05);术后1周、术后1个月,患者的NIHSS评分较术前呈显著的降低趋势,且差异具有统计学意义(P<0. 05);术后1个月复查,49例患者的MMSE、MoCA评分较术前升高,差异具有统计学意义(P <0. 05)。结论PTAS治疗缺血性脑血管病对患者认知功能、脑血流参数及神经功能均具有显著地改善作用。 Objective To investigate the effect of endovascular stenting(PTAS) on cognitive function,cerebral blood flow parameters and neurological function in patients with ischemic cerebrovascular disease. Methods 49 patients with severe internal carotid artery stenosis and symptoms treated with PTAS were enrolled in our hospital. The systolic peak blood of the middle cerebral artery(MCA) was observed before and after treatment,as well as. flow velocity(PSV),pulsatility index(PI),cerebrovascular reserve capacity(CVR),Mini-Mental State Scale(MMSE) score,Montreal Cognitive Function Scale(MoCA) score,National Institutes of Health Stroke Scale score(NIHSS) and the diameter of the carotid stenosis segment. Results 3 days after PTAS,the mean PSV,PI and CVR values of MCV were significantly improved in 49 patients compared with preoperative(P < 0. 05). The diameter of carotid artery in patients with stenosis was significantly increased compared with that before surgery(P < 0. 05). 1 week and 1 month after PTAS,the NIHSS score of patients showed a significant decrease compared with preoperative,and the difference was statistically significant(P < 0. 05). At 1 month of follow-up,MMSE and MoCA score of 49 patients were significantly higher than those before surgery,and the differences were statistically significant(P < 0. 05). Conclusions PTAS treatment of ischemic cerebrovascular disease has a significant improvement in cognitive function,cerebral blood flow parameters and neurological function.
