两种有创动脉置管方式抽取动脉血标本血气结果比较研究 被引量:2

摘要 目的研究两种有创动脉置管方式抽取动脉血标本对血气分析检测结果的影响。方法选择2017年1月至2018年5月采用有创动脉置管监测血压和(或)监测血气分析的患者30例。观察组(即A组)使用BD公司生产的一次性真空采血针从动脉置管内抽取血标本检测血气,A组又分为动脉置管抽取3ml(冲管液2ml+1ml血液)动脉血弃去再抽取血气标本检测的结果组为A1组,动脉置管抽取4ml(冲管液2ml+2ml血液)动脉血弃去再抽取血气标本检测的结果组为A2组,对照组(即B组)直接穿刺动脉抽取血气标本检测,比较两组的差异。结果观察组(A组)血标本检测的pH、PaO2结果略低于对照组(B组)血气结果,PaCO2略高于B组,但差异无统计学意义,而A1组的采血方式与A2组采血方式对pH、PaO2和PaCO2结果无影响,差异无统计学意义。结论有创动脉置管抽取3ml动脉血弃去后的血标本与直接抽取动脉血的结果一致,既保证了血气结果的准确性,又减少了血液浪费导致的医源性失血。 Objective To study the effect of two kinds of arterial blood samples taken by invasive arterial catheterization on blood gas analysis. Methods From January 2017 to May 2018, 30 patients with invasive arterial catheterization for monitoring blood pressure and/or monitoring blood gas analysis were selected. Blood gas samples were collected from arterial catheterization by one-off vacuum blood sampling needle made by BD Company as observation group (groupA). GroupAwas divided into groupA1, which was divided into arterial catheterization, extraction of 3ml of arterial blood (2ml of flushing fluid +1 ml of blood), and extraction of blood gas samples.Arterial catheterization was performed to extract 4 ml of arterial blood (2 ml of flushing fluid +2 ml of blood) and then blood gas samples were extracted. The test result group was A2. Control group (group B) directly puncture the artery to extract blood and gas samples for testing.The difference between the two groups was compared. Results The results of blood pH and PaO2 in the observation group (group A) were slightly lower than those in the control group (group B). PaCO2 was slightly higher than that in the group B, but there was no statistical difference. There was no significant difference in the results of pH, PaO2 and PaCO2 between group A1 and group A2. Conclusion The results of 3 ml artery blood discarded by invasive arterial catheterization are consistent with those of direct arterial blood extraction, which not only ensures the accuracy of blood gas results, but also reduces iatrogenic blood loss caused by blood waste.
