摘要 目的 分析肝移植术后早期患者桡动脉有创血压与上肢无创血压测量间的差异性,探讨肝移植术后患者血压监测的最佳方法.方法 选择2016年2月至2017年6月肝移植术后早期患者40例,根据桡动脉收缩压(SBP)水平分为2组,A组:SBP?90~140mmHg;B组:SBP>140mmHg,采用自身对照的方法同时测量并记录桡动脉有创血压及对侧上肢无创血压,血压数据录入SPSS?19.0软件,采用配对t检验分析.结果 当桡动脉SBP在90~140mmHg,有创与无创SBP、舒张压(DBP)差异无统计学意义(P>0.05);当桡动脉SBP>140mmHg时,有创比无创SBP平均高10mmHg,DBP平均低3mmHg,差异有统计学意义(P<0.05).结论 肝移植术后循环稳定者建议尽早拔除有创血压监测装置,以减少动脉置管相关并发症. Objective To study the the difference between radial artery invasive blood pressure and upper limb noninvasive blood pressure,and find the best way to monitor the blood pressure in the early stage after liver transplantation.Methods 40 patients after liver transplantation in department of critical care unit from February 2016 to June 2017,according to the radial artery systolic blood pressure,were divided into 2 groups:group A,systolic blood pressure(SBP)90~140mmHg;group B,SBP>140mmHg.A self-control method was used to measure and record the blood pressure of the radial artery and the noninvasive blood pressure on another side of the upper limb.All statistical analysis was performed in SPSS 19.0,and statistical significance was determined by unpaired two-tailed student t test.Results When the systolic pressure of the radial artery was 90 to 140mmHg,there was no significant difference between the systolic pressure and the diastolic pressure of invasive and noninvasive blood pressure(P>0.05).When the systolic pressure of the radial artery was more than 140mmHg,the systolic pressure of invasive blood pressure was 10mmHg higher than that of noninvasive blood pressure,while the diastolic pressure of invasive blood pressure was 3mmHg lower than that of noninvasive blood pressure(P<0.05).Conclusion It is recommended to remove the invasive blood pressure monitoring devices as soon as possible to reduce the complications of arterial catheterization after liver transplantation.
