摘要
目的探究高频超声在类风湿关节炎(RA)患者早期诊断中的应用价值。方法选取类风湿关节炎(RA)患者40例为观察组,在该院于2018年1—12月进行诊断检查治疗,同期选择40名健康者为对照组。两组均会选择高频超声及X线检查诊断治疗。对比两组滑膜增生、骨侵蚀、膝关节髌上囊积液阳性检出率,以及观察组红细胞沉降率(ESR)、C-反应蛋白(CRP)和超声检查相关指标相关性。结果相较于对照组,观察组的滑膜彩色血流(χ^2=5.622,P=0.018)、骨侵蚀(χ^2=7.963,P=0.000)、膝关节髌上囊积液(χ^2=27.585,P=0.000)、滑膜增生超声检查阳性率(χ^2=24.395,P=0.000)均显著较高,对比差异有统计学意义(P<0.05),相较于对照组,观察组的外髁厚度对比(t=11.724,P=0.000)及股骨内髁厚度对比(t=33.165,P=0.000)均显著较薄,动脉阻力指数对比(t=140.662,P=0.000)显著较小,差异有统计学意义(P<0.05);在观察组患者中检测膝关节80个,相较于X线片的检查方式,高频超声对患者滑膜增生(χ^2=4.306,P=0.038)、骨侵蚀(χ^2=6.782,P=0.009)、膝关节髌上囊积液的检出率(χ^2=4.682,P=0.030)均显著较高,两种检查方式对比差异有统计学意义(P<0.05);观察组的滑膜增生厚度(R=0.725)、彩色血流分级(r=0.661)和ESR(r=0.520)存在相关性(P<0.01);其和CRP也具备相关性(r=0.727、0.691、0.544,P<0.01)。结论针对类风湿关节炎(RA)患者,采用高频超声检查诊断治疗,对各病变检出率高于X线片,可较好反映早期患者的病变情况,且CRP、ESR和高频超声检查指标有一定相关性,并能够改善患者的生活质量,具有临床应用价值。
Objective To explore the value of high frequency ultrasound in the early diagnosis of rheumatoid arthritis(RA)patients.Methods Forty patients with rheumatoid arthritis(RA)were enrolled in the observation group.The diagnosis and treatment were performed in our hospital from January to December 2018.40 healthy subjects were selected as the control group.Both groups will choose high-frequency ultrasound and X-ray examination diagnosis and treatment.The synovial hyperplasia,bone erosion,positive rate of supracondylar sac accumulate in the knee group,and the correlation between erythrocyte sedimentation rate(ESR),C-reactive protein(CRP)and ultrasound related indicators in the observation group were compared.Results Compared with the control group,the color blood flow of the synovial membrane in the observation group(χ^2=5.622,P=0.018),the bone erosion(χ^2=7.963,P=0.000),andthe supracondylar capsule(χ^2=27.585,P=0.000),the positive rate of synovial hyperplasia(χ^2=24.395,P=0.000)was significantly higher.The difference was statistically significant(P<0.05).Compared with the control group,the contrast of the external thickness of the observation group was compared(t=11.724,P=0.000),and the thickness of the femoral condyle(t=33.165,P=0.000),which was significantly thinner.The arterial resistance index(t=140.662,P=0.000),which was significantly smaller,the difference was statistically significant(P<0.05).In the observation group,80 knee joints were detected.Compared with the X-ray examination method,the high-frequency ultrasound had a synovial hyperplasia(χ^2=4.306,P=0.038),and a bone erosion(χ^2=6.782,P=0.009).The detection rate of the knee joint sac fluid(χ^2=4.682,P=0.030).The difference between the two methods was statistically significant(P<0.05).The synovial hyperplasia in the observation group of thickness(R=0.725),color flow grading(r=0.661)and ESR(r=0.520)are correlated(P<0.01);and CRP which also includes a correlation(r=0.727,0.691,0.544,P<0.01).Conclusion For patients with rheumatoid arthritis(RA),high-fr
