剪切波弹性成像和超声造影在乳腺良恶性肿瘤鉴别中的应用比较 被引量:9

Utility of shear wave elastography versus contrast-enhanced ultrasound in identifying benign and malignant breast tumors:a comparative study

摘要 目的 比较剪切波弹性成像(SWE)和超声造影在乳腺良恶性肿瘤鉴别诊断中的应用效果。方法 选择90例乳腺肿瘤患者,均行SWE和超声造影检查。观察SWE检查时肿块的最大弹性模量值(Emax)、最小弹性模量值(Emin)、平均弹性模量值(Emean)、病灶脂肪弹性比(Eratio)及超声造影检查时肿块的达峰增强水平、增强方向、增强后肿块大小、造影剂分布均匀程度。以病理诊断结果(良性43例,恶性47例)为金标准,比较SWE和超声造影诊断乳腺恶性肿瘤的灵敏度、特异度和准确度。结果 良性组和恶性组患者的Emax、Emin、Emean和Eratio、达峰增强水平、增强后肿块大小比较,差异均有统计学意义(均P<0.05)。多因素分析结果显示,Emax和Eratio、达峰增强水平和增强后肿块大小分别是SWE及超声造影诊断乳腺恶性肿瘤的影响因素(均P<0.05)。SWE和超声造影诊断乳腺恶性肿瘤的灵敏度、特异度和准确度分别为80.85%、93.02%、86.67%和78.72%、86.05%、82.22%,差异均无统计学意义(均P>0.05)。结论 SWE和超声造影检查对鉴别乳腺良恶性肿瘤均具有较好的应用价值。 Objective To compare the application effects of shear wave elastography(SWE) versus contrast-enhanced ultrasound(CEUS) in the differential diagnosis of benign and malignant breast tumors. Methods Ninety patients with breast tumor were selected and underwent SWE and CEUS examinations.Masses′ maximum modulus of elasticity (E max ),minimum modulus of elasticity(E min ),mean modulus of elasticity(E mean ),and lesion-to-fat elasticity ratio(E ratio ) on SWE were observed,together with masses′ peak enhancement level,enhancement direction,enhanced mass size,and uniformity coefficient of contrast agent distribution on CEUS.The results of pathological diagnosis of 43 benign and 47 malignant cases served as the gold standard,SWE was compared with CEUS in items of sensitivity, specificity and accuracy for diagnosing breast malignancies. Results Statistically significant differences were observed in E max ,E min ,E mean ,E ratio ,peak enhancement level and enhanced mass size between the benign group and the malignant group(all P <0.05).Multivariate analysis results showed that E max ,E ratio ,peak enhancement level and enhanced mass size were influencing factors of SWE and CEUS separately for the diagnosis of breast malignant tumors(all P <0.05).The sensitivities,specificities and accuracies of SWE and CEUS for diagnosing breast malignancies were 80.85% and 78.72 %,93.02% and 86.05%,and 86.67% and 82.22%,respectively,presenting no statistically significant difference (all P >0.05). Conclusion Both SWE and CEUS examinations have a favorable application value in identifying benign and malignant breast tumors.
