摘要
目的评价降阶梯疗法用于新生儿细菌感染性肺炎的治疗预后效果观察。方法选择该院2017年7月—2018年8月期间收治的80例细菌感染性肺炎新生儿,随机分为接受哌拉西林他唑巴坦钠(50 mg/kg,8 h/次)治疗的对照组以及降阶梯疗法的观察组。记录新生儿体温恢复、啰音消失、呼吸平稳、住院时间、不良反应以及血清WBC、CRP、PCT水平变化情况,并比较两组新生儿临床治疗效果。结果组间用药后不良反应情况比较,差异无统计学意义(P>0.05)。组间用药后体温恢复正常、啰音消失等症状改善所需时间观察组均短于对照组,用药后WBC (血清白细胞计数)、PCT (降钙素原)、CRP (反应蛋白)水平比较观察组均优于对照组(t=4.999 8、4.999 2、12.536 2,P=0.000 0),且观察组总有效率97.50%(39/40)高于对照组87.50%(35/40),差异有统计学意义(χ^2=10.167 5,P<0.05)。结论针对新生儿细菌感染性肺炎,降阶梯疗法在保证安全性的基础上提高了治疗预后效果。
Objective To evaluate the prognostic effect of descending step therapy for neonatal bacterial infection pneumonia. Methods Eighty neonates with bacterial infection pneumonia admitted to our hospital from July 2017 to August 2018 were randomly divided into control group receiving piperacillin tazobactam sodium (50 mg/kg noce every 8 h). And an observation group for step-down therapy. The recovery of neonatal body temperature, loss of voice, steady breathing, length of hospital stay, adverse reactions, and changes in serum WBC, CRP, and PCT levels were recorded, and the clinical effects of neonates in the two groups were compared. Results The adverse reactions after treatment be-tween groups were compared,and the difference was not statistically significant (P>0.05). After the treatment, the body temperature returned to normal, the voice disappeared and other symptoms improved. The observation group was shorter than the control group. The WBC (serum white blood cell count), PCT (procalcitonin) and CRP (reactive protein) levels were compared after treatment, superior to the control group(t=4.999 8, 4.999 2, 12.536 2, P=0.000 0), and the total ef-fective rate of the observation group was 97.50%(39/40) higher than the control group 87.50%(35/40),the difference was statistically significant (χ^2 =10.167 5, P<0.05). Conclusion For neonatal bacterial infection pneumonia, step-down therapy improves the prognosis of patients on the basis of ensuring safety.
