摘要
目的:探讨百令胶囊结合盐酸氨溴索对COPD合并肺部感染ICU患者肺功能及血清NF-κB、TNF-α、IL-8等因子的影响。方法:选择COPD合并肺部感染ICU患者120例,采用随机数字表法分为对照组和观察组各60例。所有患者视病情给予常规对症治疗,在此基础上,对照组患者给予盐酸氨溴索治疗,观察组患者给予百令胶囊结合盐酸氨溴索治疗,比较两组患者治疗前后肺功能指标,血清NF-κB、TNF-α、IL-8变化及临床疗效。结果:治疗前,两组患者FEV1、FEV1/FVC、FEV1%Pre肺功能指标,血清NF-κB、TNF-α、IL-8水平比较差异均无统计学意义(P>0.05);治疗后,两组患者FEV1、FEV1/FVC、FEV1%Pre指标均升高,血清NF-κB、TNF-α、IL-8水平降低(P<0.05);观察组治疗后FEV1、FEV1/FVC、FEV1%Pre均高于对照组,血清NF-κB、TNF-α、IL-8水平均低于对照组(P<0.05)。观察组患者总有效率为95.00%,高于对照组的83.33%,差异有统计学意义(P<0.05)。结论:百令胶囊结合盐酸氨溴索可显著改善COPD合并肺部感染的ICU患者肺功能,降低气道炎症反应状态,疗效确切,值得临床进一步推广和应用。
Objective:To investigate the effects of Bailing capsule combined with ambroxol hydrochloride on pulmonary function and serum levels of NF-Kappa B,TNF-alpha and IL-8 in patients with COPD complicated with pulmonary infection in ICU.Methods:120 patients with COPD complicated with pulmonary infection in ICU admitted to our hospital from February 2014 to January 2019 were selected.Random number table method was used to divide the patients into control group and observation group,60 cases each.All patients were given routine symptomatic treatment according to their condion,while the control group was given ambroxol hydrochloride,while the observation group was given Bailing capsule combined with ambroxol hydrochloride.The changes of pulmonary function,serum levels of NF-Kappa B,TNF-alpha and IL-8 and clinical efficacy were compared between the two groups before and after treatment.Results:Before treatment,there was no significant difference in FEV1,FEV1/FVC,FEV1 % Pre pulmonary function index,serum levels of NF-Kappa B,TNF-alpha and IL-8 between the two groups (P>0.05);after treatment,the levels of FEV1,FEV1/FVC and FEV1 % Pre increased,while the levels of serum NF-Kappa B,TNF-alpha and IL-8 decreased in the observation group (P<0.05).The levels of serum NF-Kappa B,TNF-alpha and IL-8 in the observation group were lower than those in the control group (P<0.05),and the total effective rate in the observation group was 95.00% higher than 83.33% in the control group (P<0.05).Conclusion :Bailing capsule combined with ambroxol hydrochloride can significantly improve the pulmonary function of ICU patients with COPD complicated with pulmonary infection,and reduce the airway inflammation.The curative effect is remarkable,which is worthy of further promotion and application in clinic.
