摘要
目的分析纹带棒状杆菌感染的临床特征及对常用抗生素的耐药情况,为临床合理用药提供实验室依据。方法收集2017年3月~2018年12月临床标本中分离的纹带棒状杆菌42株,鉴定采用法国生物梅里埃公司VITEK-2 compact全自动微生物鉴定仪及配套的厌氧菌-棒状杆菌(ANC)鉴定卡。用微量肉汤稀释法检测20种抗生素的最低抑菌浓度(MIC)。结果42株纹带棒状杆菌主要来源为下呼吸道标本(83.3%),分离率以老年病科最高(35.7%)。万古霉素、利萘唑胺及利福平对纹带棒状杆菌有较高的敏感率,庆大霉素、红霉素和阿奇霉素等次之,而β-内酰胺酶类抗生素对纹带棒状杆菌耐药率都在90%以上,其他抗生素如亚胺培南、美罗培南、环丙沙星、左氧氟沙星、克林霉素和四环素等敏感率较低。结论纹带棒状杆菌在临床标本中的分离率增高,多侵犯免疫力低下人群,分离株多为多药耐药菌株,万古霉素、利萘唑胺和利福平为临床首选用药。
Objective To analyzed the clinical characteristics of Corynebacterium striatum infection and its resistance to commonly used antibiotics,so as to provide laboratory evidence for rational drug therapy.Methods A total of 42 strains of C.striatum were collected from clinical specimens from Mar.2017 to Dec.2018.The identification was carried out by VITEK-2 compact automatic microbial identification instrument and ancillary anaerobic-corynebacterium(ANC)identification card.Minimum inhibitory concentration(MIC)of 20 antimicrobial agents was determined by broth microdilution method.Results Most of the 42 Corynebacterium striatum strains were lower respiratory tract specimens(83.3%),with the highest detection rate(35.7%)in geriatrics.Vancomycin,linezolid and rifampicin were more sensitive to C.striatum,followed by gentamicin,erythromycin and azithromycin,while beta-lactamase antibiotics were more than 90%resistant to C.striatum.Other antibiotics such as imipenem,meropenem,ciprofloxacin,levofloxacin,clindamycin and tetracycline were also less sensitive.Conclusion Corynebacterium striatum isolation rate in clinical specimens increased,mostly invaded the people with low immunity.Most isolates were multidrug-resistant strains.Vancomycin,linezolid and rifampicin would be the first choice for clinical treatment.
