摘要
目的探讨分阶段体位管理结合自主屏气用力应用于初产妇第二产程中的效果。方法选择2018年1月至2018年6月无锡市妇幼保健院接收的初产妇60例作为研究对象(观察组),于产妇进入第二产程后行分阶段体位管理结合自主屏气用力指导的干预措施,另以2017年7月至2017年12月接收的行传统接产方式的初产妇60例作为对照组,对比两组产妇第二产程时间、自主用力时间、分娩方式、新生儿窒息发生率、产后2h出血量、自然分娩产妇会阴保护情况及分娩满意度。结果观察组产妇的第二产程时间、自主用力时间短于对照组,自然分娩率高于对照组,新生儿窒息发生率低于对照组,产后2h出血量低于对照组,产妇分娩满意度高于对照组,差异均有统计学意义(均P<0.05);观察组自然分娩产妇会阴保护情况也优于对照组(P<0.05)。结论分阶段体位管理结合自主屏气用力干预措施应用于初产妇第二产程中可显著缩短产程时间和自主用力时间,减少产后出血量,改善分娩结局,提高分娩满意度。
Objective To explore the effect of staged position management combined with independent breath holding in the second stage of primipara. Methods Sixty primiparae admitted by Wuxi Maternal and Child Health Hospital from January 2018 to June 2018 were selected as the study subjects (observation group).After entering the second stage of labor,60 primiparae receiving traditional delivery methods from July 2017 to December 2017 were selected as the study group.The labor time of second stage,length of self-exertion,mode of delivery,incidence of neonatal asphyxia,2 hours postpartum hemorrhage,perineal protection and delivery satisfaction were compared between the two groups. Results The length of second stage of labor and self-exertion time in the observation group were shorter than those of the control group.The natural delivery rate was higher than that of the control group.The incidence of neonatal asphyxia in observation group was lower than that of the control group.The amount of postpartum hemorrhage was lower than that of the control group.And the maternal satisfaction with delivery was higher than that of the control group (all P <0.05 ).The effect of perineum protection was also better than that of control group ( P <0.05 ). Conclusions The application of staged posture management combined with independent breath holding intervention in the second stage of primipara can significantly shorten the duration of labor and self-control time,reduce postpartum hemorrhage,improve the outcome and satisfaction of delivery.
