摘要
目的 观察评价分析磁珠法在提取核酸HIV-RNA检测中的应用效果。方法 选择2017年7月至2019年3月期间中山市第二人民医院爱心门诊经确诊艾滋病毒感染并接受抗病毒治疗的患者100例,运用磁珠法分离提取纯化核酸,荧光定量PCR检测系统及柱提取核酸荧光定量PCR检测系统测定病毒核酸HIV-RNA载量;随机对照试验分为两组,对患者的血清标本进行检测,对照组50例给予酶联免疫吸附测定法(ELISA)检测HIV抗体,观察组50例给予磁珠法检测HIV抗体,并比较两组患者的检测准确率、敏感度及特异性。结果 观察组的检测准确率为96.00%,显著高于对照组的68.00%(χ^2=13.2791,P=0.0000);观察组检测敏感度与特异性分别为100.00%、100.00%,显著高于对照组的21.43%、18.18%(χ^2=36.2353、30.4615,均P<0.05)。结论 运用磁珠法分离提取纯化核酸,荧光定量PCR检测系统及柱提取核酸荧光定量PCR检测系统测定病毒核酸HIV-RNA载量,在艾滋病血清HIVRNA检测中,磁珠法的临床检测准确率、特异性、敏感度得到大幅度提高,在检测操作时方便、快捷、高效,值得临床检验科医师大力推广应用。
Objective To observe and evaluate the effect of applying magnetic bead extraction in the detection of nucleic acid HIV-RNA. Methods From July, 2017 to March, 2019, 100 patients with HIV infection and taking antiviral treatment were diagnosed in the Love Clinic of our hospital. The magnetic bead method was used to separate and purify the nucleic acid, and the quantitative PCR detection system was used. The nucleic acid fluorescent quantitative PCR detection system was used to determine the viral nucleic acid HIV-RNA load;the patients were divided into a control group and an observation group, 50 cases for each group. The patients' serum samples were tested. The control group’ HIV antibodies were detected by enzyme-linked immunosorbent assay (ELISA), and the observation group’s by magnetic beads. The detection accuracies, sensitivities, and specificities of the two groups were compared. Results The detection accuracy, sensitivity, and specificity were significantly higher in the observation group than in the control group [96.00% vs. 68.00%,χ^2=13.279 1, P=0.000 0;100.00% vs. 21.43%,χ^2=36.235 3, P < 0.05;and 100.00% vs. 18.18%,χ^2=30.461 5, P < 0.05). Conclusion Magnetic bead extraction and purification of nucleic acids, fluorescence quantitative PCR detection system, and column extraction nucleic acid fluorescence quantitative PCR detection system for the determination of viral nucleic acid HIV-RNA load, in the AIDS (AIDS) serum HIV-RNA detection, magnetic bead method has high detection accuracy, specificity, and sensitivity, and is convenient, fast, and efficient in the detection operation. It is worthy of popularization and application by clinical laboratory doctors.
