摘要
目的 探讨小针刀肛门内括约肌侧切术在重度混合痔患者中的疗效及对血管内皮细胞生长因子(VEGF)的影响。方法选取2017年11月至2018年11入住我院进行治疗的112例重度混合痔患者,按手术方法分为对照组与观察组,每组56例。2组患者均进行外剥内扎法治疗,对照组辅以开放内括约肌侧切,观察组辅以小针刀侧切。结果2组患者手术治疗前VEGF水平差异无统计学意义(P>0.05),术后VEGF水平高于术前,且观察组显著高于对照组(P<0.05);观察组患者术中出血量、手术时间、创面愈合时间、住院时间等临床手术相关指标均优于对照组(P<0.05);观察组术后VAS评分低于对照组,并且术后首次排便时间短于对照组(P<0.05);观察组临床治疗效果优于对照组,2组比较差异有统计学意义(P<0.05)。结论对重度混合痔患者施以小针刀肛门内括约肌侧切术治疗效果理想,疼痛程度小,并发症少,利于患者预后。
Objective To investigate the effect of anal internal sphincter lateral incision with small needle knife on the patients with severe mixed hemorrhoids,and to explore the effect on vascular endothelial growth factor.Methods A total of 112 patients with severe mixed hemorrhoids admitted to our hospital from November 2017 to November 2018 were selected and divided into the control group and the observation group according to surgical methods,with 56 cases in each group.Both of the two groups were treated with external stripping and internal ligation.The control group was supplemented by open internal sphincter lateral incision,and the observation group was supplemented by small needle knife lateral incision.Results There was no significant difference in VEGF level between the two groups before surgery(P>0.05).The VEGF level of the two groups were increased after operation,and the observation group was significantly higher than the control group(P<0.05).The clinical surgical indicators such as peroperative volume,operation time,wound healing time and hospitalization in the observation group were better than those in the control group(P<0.05).The VAS score of the observation group was lower than that of the control group,and the first defecation time of the observation group was shorter than that of the control group(P<0.05).The clinical treatment effect of the observation group was significantly better than that of the control group(P<0.05).Conclusion Anal internal sphincter lateral incision with small needle knife for the patients with severe mixed hemorrhoids is effective,and it is conducive to the prognosis of patients with less pain and complications.
