摘要
目的对不同机械通气方式对新生儿呼吸窘迫综合征的疗效进行分析和评价。方法选择该院在2017年1月-2018年1月期间收治的80例新生儿呼吸窘迫综合征患儿作为研究对象,按照病历单双号将其随机划分为对照组和观察组,每组各有40例患儿。采用常频振荡通气方式治疗对照组,采用高频振荡通气方式治疗观察组,对两组患儿的治疗效果进行分析和比较。结果对照组患儿治疗前的PaO2为(59.5±5.6)mm Hg,PaCO2为(49.1±4.5)mm Hg,观察组治疗前的PaO2为(59.3±5.8)mmHg,PaCO2为(50.9±5.3)mmHg,两组患儿治疗前的血气指标均差异无统计学意义(P>0.05)。对照组患儿治疗24 h后的PaO2为(73.4±5.1)mmHg,PaCO2为(46.3±1.8)mm Hg,观察组治疗24 h后的PaO2为(79.8±6.7)mmHg,PaCO2为(40.1±1.1)mm Hg,两组患儿治疗24 h后的血气指标差异有统计学意义(P<0.05)。对照组患儿并发症发生率为17.5%,观察组并发症发生率为2.5%。在并发症发生率方面两组患儿相比差异有统计学意义(χ2=5.428,P<0.05)。结论在新生儿呼吸窘迫综合征的临床治疗中高频振荡机械通气具有很好的治疗效果,能够使患儿的肺功能得到有效改善。
Objective To analyze and evaluate the effect of different mechanical ventilation methods on neonatal respiratory distress syndrome. Methods Eighty children with neonatal respiratory distress syndrome admitted to our hospital from January 2017 to January 2018 were selected as subjects. They were randomly divided into control group and observation group according to their medical records, with 40 children in each group. The control group was treated with constant frequency oscillatory ventilation, and the observation group was treated with high frequency oscillatory ventilation. The therapeutic effects of the two groups were analyzed and compared. Results The PaO2 before treatment was(59.5±5.6)mmHg in the observation group and(59.3±5.8)mmHg in the control group.PaCO2 was(49.1±4.5)mmHg in the observation group, and(50.9±5.3)mmHg in the control group. There was no significant difference in blood gas index before treatment between the two groups(P>0.05). In the control group, the PaO2 was(73.4±5.1)mmHg after 24 hours of treatment, and the PaCO2 was(46.3±1.8)mmHg. In the observation group, the PaO2 was(79.8±6.7)mmHg after treatment for 24 hours, PaCO2 was(40.1±1.1)mmHg. The difference between the two groups in the blood gas index after treatment for 24 h was significantly different(P<0.05). The incidence of complications in the control group was 17.5%, and was2.5% in the observation group. The difference between the two groups was statistically significant(χ2=5.428, P<0.05).Conclusion High-frequency oscillatory mechanical ventilation has a good therapeutic effect in the clinical treatment of neonatal respiratory distress syndrome, which can effectively improve the lung function of children.
