摘要
目的:探讨腔内介入手术在下肢动脉硬化闭塞症患者中的临床效果及对血清炎性因子水平的影响。方法:选择2016年5月-2018年11月治疗的下肢动脉硬化闭塞症患者100例作为对象,随机分为对照组(n=50)和观察组(n=50)。对照组给予传统动脉旁路手术治疗,观察组采用腔内介入手术治疗,术后5 d对患者效果进行评估,比较两组Rutherford分级、炎症因子水平及术后并发症。结果:观察组术后5 d的Rutherford分级中0+1级比例高于对照组(P<0.05);观察组术后5 d的Rutherford分级2+3+4+5+6级比例,低于对照组(P<0.05);观察组术后5 d的炎症因子IL-1、IL-6及TNF-α水平低于对照组(P<0.05);观察组与对照组术后感染、血栓形成、胃肠道出血、患肢肿胀及肾功能损害发生率比较差异均无统计学意义(P>0.05)。结论:腔内介入手术用于下肢动脉硬化闭塞症患者中能改善患者Rutherford分级,降低炎症因子水平,未增加术后并发症发生率,值得推广应用。
Objective:To investigate the clinical effects of endovascular interventional surgery in patients with lower extremity arteriosclerosis obliterans and the effect on serum inflammatory factors.Method:One hundred patients with lower extremity arteriosclerosis obliterans treated from May 2016 to November 2018 were enrolled and randomly divided into the control group(n=50) and the observation group(n=50).The control group was treated with conventional arterial bypass surgery.The observation group was treated with endovascular intervention.The effect of the patients was evaluated 5 days after surgery.The Rutherford classification,inflammatory factor levels and postoperative complications were compared.Result:The rates of grades 0+1 in the Rutherford grade of the observation group were higher than those of the control group(P<0.05).The rates of Rutherford grade 2+3+4+5+6 of the observation group were lower than those of the control group(P<0.05).The levels of inflammatory factors IL-1,IL-6 and TNF-α in the observation group were lower than those in the control group(P<0.05).There were no significant differences in the postoperative infection,thrombogenesis,the incidence of intestinal bleeding,limb swelling and renal dysfunction(P>0.05).Conclusion:Endovascular intervention for lower extremity arteriosclerosis obliterans can improve the Rutherford classification,reduce the level of inflammatory factors,and the incidence of postoperative complications without increase.It is worthy of popularization and application.
