内膜下血管成形术治疗下肢动脉闭塞症的临床疗效观察 被引量:3

摘要 目的探讨内膜下血管成形术(subintimal angioplasty,SIA)治疗下肢动脉闭塞症的临床价值及疗效。方法采用内膜下成形术治疗43例长段髂动脉、股浅动脉闭塞患者,对手术成功率、并发症发生率、初始通畅率、后期随访通畅率进行分析。结果 43例SIA成功41例,技术成功率95.3%,术后6、12、24、36个月的初始通畅率分别为97.6%、84.2%、68.8%和53.8%。41例中无截肢患者,保肢率100%。术中、术后无严重手术并发症发生,2例动脉穿孔,未发生严重后果;1例术中血栓形成;术后2例穿刺部位血肿,1例假性动脉瘤形成,经局部压迫后消失。结论 SIA是治疗长段髂动脉、股浅动脉闭塞的有效方法,中期随访效果满意。 Objective To investigate the clinical value and curative effect of subintimal angioplasty(SIA) for the treatment of atherosclerosis obliterans in the lower extremities. Methods Fourty three patients with long segment occlusion of iliac artery and superifcial femoral artery undewent SIA.The successful rate, complication rates, primary patency rates, patency rates of follow-up of the operative procedure were analyzed. Results Of 43 cases undergoing SIA,41 were successful and the technical successful rate was 95.3%.The primary patency rate in 41 cases at 6,12,24 and 36 operative monthes was 97.6%,84.2%,68.8%and 53.8%respectively.There were no cases on amputation in 41 cases and the limb salvage rate was 100%.There was no serious complication during perioperative period.Of 41 cases 2 have artery perforation, without serious results,1 has artery thrombosis during the operation,2 do hematoma post the operation and 1 does pseudoaneurysm with covering by means of regional compression . Conclusions SIA is an effective procedure for the treatment of long segment occlusion of iliac artery and superifcial femoral artery and the followed-up results of midterm are satisfying.
