摘要
目的提高签约居民在家庭医生处首诊比例,构建分级诊疗的工作基础。方法推行“三优化”模式,夯实服务基础;实施药品“两多两送”,落实签约服务优势政策;依托绩效与考核“两个抓手”,努力提高服务内涵与品质。结果签约居民家庭医生就诊率、家庭医生门诊签约居民就诊率、签约居民家庭医生延处方率分别为39.64%、29.70%、81.43%;家庭医生服务知晓率方面,对应就诊的居民远高于非对应就诊的居民,P<0.01。在服务满意度方面,对应就诊的居民同样远高于非对应就诊的居民,P<0.01。结论基于提高签约后有效服务的家庭医生运行机制改革探索,立足于提高签约居民在家庭医生处首诊比例,取得了良好的成绩,是社区卫生综合改革一次重要的实践探索。
Objective:To increase the proportion of contracted residents who receive their first diagnosis from family doctors,and build a working basis for hierarchical diagnosis and treatment.Method:“Three optimization”has been carried out to consolidate the service foundation.The policies of“two more and two free delivery”of drugs,and of advantage in contract services will be implemented.Relying on the“two handles”of performance and assessment,the connotation and quality of service will be improved.Result:The consultation rate,the outpatient consultation rate,and the delayed prescription rate of contracted residents’family doctors were 39.64%,29.70%and 81.43%respectively.In terms of the awareness rate of family doctor services,the residents who received corresponding treatment were much higher than those who did not,P<0.01.In terms of service satisfaction,residents responding to medical treatment are also much higher than those not responding to medical treatment,P<0.01.Conclusion:Based on the reform of the operating mechanism of family doctors to improve the effective service after signing up,and the increase of the proportion of residents signing up for the first consultation with family doctors,good results have been achieved,which is an important practical exploration of comprehensive community health reform.
