摘要
团队机制是社区慢性病管理的新模式,近几年我社区实践的结果充分表明在社区建立一支分工明确、优势互补的高效全科团队,可以有效缓解社区人力资源紧缺、任务繁重的困境,使社区慢性病管理在团队模式下向着更加规范、有序、有效的方向发展。在社区卫生服务蓬勃发展的今天,建立一支高素质、高效率的全科团队,刻不容缓。
General practice(GP)team is a new model in community-based chronic disease management.In recent years,the practice of our station has showed that establishment of a high-performance GP team could effectively alleviate the shortage of human resources and predicament of heavy task in community health services,so as to make the development of community-based chronic disease management more standardized,orderly and effective.It is urgent to establish a GP team with high-quality and efficiency today when the community health service is flourishing.
