摘要
目的研究超敏C反应蛋白(hs-CRP)、降钙素原(PCT)对新生儿败血症的诊断价值及与患儿病情的相关性。方法回顾性分析2015年3月至2018年9月该院收治的100例新生儿败血症患儿(败血症组)临床资料,并随机抽取100例同时期非感染性疾病患儿作为对照组,比较2组患儿血清hs-CRP、PCT水平。采用诊断试验四格表评价hs-CRP、PCT诊断效能,采用Pearson相关性分析评价hs-CRP、PCT水平与全身感染相关性器官功能衰竭评分(SOFA)及患儿危重病例评分(PCIS)相关性。结果败血症组血清hs-CRP水平及阳性率、PCT水平及阳性率均显著高于对照组,差异均有统计学意义(P<0.05)。hs-CRP阳性结果判断新生儿败血症的敏感度、特异度、准确度为55.00%、76.00%、65.50%,PCT阳性结果判断新生儿败血症的敏感度、特异度、准确度为76.00%、95.00%、85.50%,hs-CRP阳性+PCT阳性判断新生儿败血症的敏感度、特异度、准确度为95.00%、73.00%、84.00%;血清hs-CRP、PCT水平与SOFA评分、PCIS评分均呈正相关(P<0.05)。结论新生儿败血症患儿血清hs-CRP、PCT水平显著高于非感染性疾病患儿,血清hs-CRP、PCT联合检测可以用于新生儿败血症的早期诊断及病情评估。
ObjectiveTo study the diagnostic value of high-sensitivity C-reactive protein(hs-CRP)and procalcitonin(PCT)for neonatal sepsis and its correlation with the condition of children.MethodsThe clinical data of 100 children with sepsis(septic group)admitted to the hospital from March 2015 to September 2018 were retrospectively analyzed.100 children with non-infectious diseases at the same time were randomly selected as the control group.Serum hs-CRP and PCT levels were compared between the two groups.The diagnostic efficacy of hs-CRP and PCT was evaluated using a four-dimensional table of diagnostic tests.Pearson correlation analysis was used to evaluate the correlation between hs-CRP and PCT levels and systemic infection-related organ failure score(SOFA)and critical illness score(PCIS).ResultsSerum hs-CRP levels,positive rate,PCT level and positive rate in the sepsis group were significantly higher than those in the control group(P<0.05).The sensitivity,specificity and accuracy of hs-CRP positive results for juvenile sepsis were 55.00%,76.00%,and 65.50%.The sensitivity,specificity and accuracy of PCT positive results in judging neonatal sepsis were 76.00%,95.00%,and 85.50%.The sensitivity,specificity and accuracy of hs-CRP positive+PCT positive judgment of neonatal sepsis were 95.00%,73.00%,and 84.00%.Serum hs-CRP and PCT levels were positively correlated with SOFA score and PCIS score(P<0.05).ConclusionSerum hs-CRP and PCT levels are significantly higher in children with neonatal sepsis than in non-infectious diseases.Serum hs-CRP and PCT combined detection can be used for early diagnosis and assessment of neonatal sepsis.
