摘要
目的:研讨降钙素原(PCT)和C反应蛋白(CRP)水平差值在早产儿败血症中的诊断价值。方法:选取2017年1月-2022年1月揭阳市人民医院收治的88例败血症早产儿作为观察组,另选取同期88例无败血症的早产儿作为对照组。分别在入院当天(d_(0))、入院1 d(d_(1))、入院2 d(d_(2))、入院3 d(d_(3))检测两组的PCT、CRP、白细胞(WBC)水平。结果:两组CRP d_(2)、CRP d_(3)、△WBC d_(0-1)、WBC d_(2)、WBC d_(3)比较,差异无统计学意义(P>0.05);观察组△PCT d_(0-1)、PCT d_(2)、PCT d_(3)、△CRP d_(0-1)高于对照组,差异有统计学意义(P<0.05)。观察组中,80例患儿的PCT在治疗3 d后恢复正常,8例患儿因病情加重转院退出研究。△PCT d_(0-1)诊断早产儿败血症的特异度与敏感度均高于△CRP d_(0-1)。结论:△PCT与△CRP均能实现新生儿细菌感染的诊断鉴别,△PCT诊断新生儿败血症的价值更高,且变化幅度越大说明病情越严重。
Objective:To study the diagnostic value of the difference between procalcitonin(PCT)and C-reactive protein(CRP)levels in premature infants with sepsis.Methods:From January 2017 to January 2022,88 premature infants with sepsis in Jieyang People’s Hospital were selected as the observation group,and 88 premature infants without sepsis during the same period were selected as the control group.The levels of PCT,CRP and white blood cells(WBC)in the two groups were detected on the day of admission(d_(0)),Day 1 of admission(d_(1)),Day 2 of admission(d_(2)),and Day 3 of admission(d_(3)),respectively.Results:There were no significant differences between two groups in CRP d_(2),CRP d3,△WBC d_(0-1),WBC d_(2)and WBC d_(3)(P>0.05).△PCT d_(0-1),PCT d_(2),PCT d_(3)and△CRP d_(0-1) in the observation group were higher than those in the control group,and the differences were statistically significant(P<0.05).In the observation group,PCT in 80 children returned to normal after 3 days of treatment,and 8 children were transferred to other hospital due to the condition exacerbation and discontinued from the study.The specificity and sensitivity of△PC d_(0-1)for the diagnosis of sepsis in preterm infants were higher than those of△CRP d_(0-1).Conclusion:Both△PCT and△CRP can realize the diagnosis and identification of neonatal bacterial infection,△PCT has a higher value in diagnosing neonatal sepsis,and the greater change indicates more serious disease.
