摘要
目的探讨外伤导致虹膜损伤合并白内障、晶状体脱位或无晶状体眼,行瞳孔成形联合人工晶状体植入术的手术技巧及临床疗效。方法回顾性分析我院自2016年至2018年诊治的因眼外伤导致的伴有虹膜损伤瞳孔异常合并白内障、晶状体脱位、无晶状体眼等24只眼的患者,行瞳孔成形联合白内障摘出人工晶状体植入和(或)人工晶状体缝线固定并前段玻璃体切除术。结果23人术后视力均得到不同程度提高,瞳孔居中、接近圆形,畏光和炫光程度均得到明显改善,1人发生视网膜脱离,2人术后眼压升高,药物治疗后眼压正常。结论瞳孔成形联合人工晶状体植入术,在提高视力的同时,能消除畏光、眩光症状,最大程度适应自然光线,提高视觉质量。
Objective To investigate the surgical technique and clinical effect of coreoplasty combined with intraocular lens implantation in patients with iris damage with traumatic cataract, lens luxtion or aphakia. Methods A retrospective analysis was conducted of 28 eyes with iris damage, traumatic cataract, lens luxtion or aphakia due to ocular trauma in our hospital from 2016 to 2018. Coreoplasty combined with cataract extraction, intraocular lens implantation/intraocular lens suture fixation and anterior vitrectomy were performed. Results Visual acuity were improved to different degrees after the surgery for 23 patients, the pupil was located in the middle and had a nearly circular appearance, photophobia and glare were also reduced. One patient developed retinal detachment. The intraocular pressure of two patients increased after the surgery, which returned to normal after medical treatment. Conclusions Coreoplasty combined with intraocular lens implantation can improve visual acuity, reduce photophobia and glare symptoms, making it possible for patients to adapt to the natural light and improve visual quality to the maximum extent.
