摘要
目的探讨围手术期健康管理护理干预对阻塞性睡眠呼吸暂停综合征(OSAS)伴痛风性关节炎患者术后生活质量的影响。方法收集耳鼻喉头颈外科住院治疗的OSAS伴痛风患者。将符合纳入标准的OSAS伴痛风患者67例随机分为观察组(n=35)和对照组(n=32)。对照组给予常规护理,观察组在常规护理的基础上给予健康管理。比较两组治疗前后焦虑和抑郁的情况,睡眠呼吸暂停生活质量指数(SAQLI)及术后患者痛风发作情况。结果采用《医院焦虑抑郁量表》评估患者焦虑和抑郁情况,治疗后观察组焦虑和抑郁情绪评分均明显低于对照组,差异均有统计学意义(P<0.05);观察组在日常生活、情绪、SAQLI方面以及术后1个月内痛风发作率优于对照组,差异有统计学意义(P<0.05)。结论个体化健康管理应用在痛风性关节炎患者的护理中,不仅能够改善患者焦虑和抑郁状态,还可明显改善SAQLI,降低术后痛风性关节炎发作率。
Objective To investigate the effects of perioperative health management on postoperative quality of life in patients with obstructive sleep apnea syndrome(OSAS) and gouty arthritis.Methods OSAS patients with gout who were hospitalized in our department were collected. Sixty-seven OSAS patients with gout who met the inclusion criteria were randomly divided into experimental group(35 patients) and control group(32 patients). Patients in control group received routine nursing care while patients in experimental group received extra health management on the basis of routine care. The anxiety and depression level, the Sleep Apnea Quality of Life Index(SAQLI) and postoperative gout attack were compared between the two groups both before and after nursing care. Results Hospital Anxiety and Depression Scale was applied to evaluate the anxiety and depression level of the patients. It was significantly lower in experimental group after nursing care than control group(P<0.05). The daily life and mood of patients, the SAQLI score and the occurrence rate of gout one month after surgery were significantly improved in experimental group when compared to the control group(P<0.05). Conclusion Application of individualized health management on patients with gouty arthritis can significantly improve the anxiety and depression levels, and the quality of life;and reduce the postoperative occurrence of gouty arthritis.
