摘要
目的探究何氏生骨散外敷联合口服药物治疗胫腓骨骨折的临床效果。方法选取本院2016年6月至2018年6月收治的200例胫腓骨骨折患者为研究对象,按照随机数表法将其分为观察组和对照组,每组各100例。对照组患者口服阿法骨化醇片和接骨七厘片,观察组患者在上述药物治疗基础上外敷何氏生骨散。比较两组患者治疗前和治疗4个月后的中医症状积分、视觉模拟评分法(visual analogue scale,VAS)评分、患肢肿胀度、血液流变学参数[全血低切黏度(low-shear wholeblood viscosity,LWBV)、血细胞比容(hematocrit,Hct)、红细胞沉降率(erythrocyte sedimentation rate,ESR)]。治疗4个月后,比较两组患者小腿功能、骨折愈合情况和并发症发生情况。结果治疗4个月后,两组患者肿痛、瘀斑、肢体萎软中医症状积分、VAS评分、患肢肿胀度、LWBV、Hct、ESR均显著低于本组治疗前(均P < 0.05),且观察组患者上述指标水平均显著低于同期对照组(均P < 0.05)。观察组患者优良率显著高于对照组(P < 0.05),骨折未获得临床愈合患者占比显著低于对照组(P < 0.05),已获得临床愈合患者的骨折愈合时间均显著短于对照组(P < 0.05)。治疗后4个月内,观察组患者并发症总发生率显著低于对照组(P < 0.05)。结论何氏生骨散外敷联合口服药物治疗能有效改善胫腓骨骨折患者的临床症状,促进骨折愈合和小腿功能恢复,有利于改善患者中远期预后,提高生活质量。
Objective To explore the clinical effect of topical application of He's Shenggu powder combined with oral drugs in the treatment of tibiofibular fracture. Method 200 patients with tibiofibular fractures treated in our hospital from June 2016 to June 2018 were selected in the study and were divided into observation group and control group according to the random number table method, with 100 cases in each group. Control group patients were given oral alfacalcidol tablets and Jiegu Qili pian, and observation group patients were treated with He's Shenggu powder on the basis of the above drug treatment. The TCM symptoms scores, visual analogue scale (VAS) score, swelling degree of affected limb, and hemorheological paramet [low-shear whole blood viscosity (LWBV), hematocrit (Hct), erythrocyte sedimentation rate (ESR)] were compared between the two groups before treatment and after 4 months of treatment. After 4 months of treatment, the calf function, fracture healing and complications were compared between the two groups. Result After 4 months of treatment, the scores of swelling and pain, ecchymosis, limb weakness, VAS score, swelling degree of affected limbs, LWBV, Hct and ESR in the two groups were significantly lower than those before treatment (all P < 0.05), the levels of the above indexes in observation group were significantly lower than those in control group (all P < 0.05). The excellent and good rate in observation group was significantly higher than that in control group (P < 0.05), the proportion of patients with unhealed fracture in observation group was significantly lower than that in control group (P < 0.05), the fracture healing time of the patients with clinical healing was significantly shorter than that of control group (P < 0.05). Conclusion He's Shenggu powder topical application combined with oral drug treatment can effectively improve the clinical symptoms of patients with tibiofibular fracture, promote fracture healing and leg function recovery, improve the medium and long term prognosis and
