摘要
目的:观察牙周基础治疗在广泛型侵袭性牙周炎治疗中的长期临床疗效,为牙周炎的临床治疗提供依据。方法:纳入24名广泛型侵袭性牙周炎患者,采用超声龈下刮治器械治疗,联合使用牙周袋内缓释剂和全身抗生素。牙周治疗后12周,每3~6个月行牙周维持治疗。比较基线与治疗后6周、12周和1年牙周探诊深度(probing depth,PD)、牙周附着水平(cliniacal attachment loss,CAL)、和出血指数(bleeding index,BI)的变化。结果:与基线相比,牙周治疗后6周、12周及1年的PD、CAL和BI值均有明显下降,差异均有统计学意义(P<0.01);治疗后12周,相较治疗后6周,PD变化有显著下降,差异有统计学意义(P<0.05);治疗后1年相较治疗后12周,PD、CAL和BI差异均无统计学意义(P>0.05)。结论:针对广泛型侵袭性牙周炎,规范彻底的牙周基础治疗安全有效;定期牙周维持治疗对牙周炎的长期疗效起重要作用。
Objective:To evaluate the long-term clinical efficacy of periodontal initial therapy in the treatment of generalized aggressive periodontitis(GAgP),and to provide evidences for clinical treatment of periodontitis.Methods:Twenty-four GAgP patients were chosen,that treated with ultrasonic scaler,combined with slow-release preparation and systemic application of antibiotics.Supportive periodontal therapy(SPT)was performed every 3 to 6 months after periodontal therapy.Periodontal probing depth(PD),clinical attachment loss(CAL)and bleeding index(BI)were recorded at baseline,6 w,12 w and 1 year after periodontal therapy.Results:At 6 w,12 w,and 1 year after periodontal initial therapy,there was a very significant decline in PD,CAL,and BI compared with the baseline respectively(P<0.01).At 12 w after treatment,the change of PD was significantly lower than that at 6 w after treatment(P<0.05).There were no significant differences in PD,CAL and BI between the 12 w and 1 year after treatment respectively(P>0.05).Conclusion:Periodontal initial therapy showed significant clinical effect in treatment of generalized aggressive periodontitis.Regular SPT is significantly important for the long-term curative outcome in the therapy for GAgP patients.
