摘要
目的观察补肾活血方治疗肾虚血瘀型早发性卵巢功能不全患者的临床疗效。方法将40例肾虚血瘀型早发性卵巢功能不全患者随机分为治疗组和对照组,每组20例。治疗组给予补肾活血方口服,对照组给予雌孕激素序贯疗法治疗,2组均连续治疗3个月经周期。观察2组治疗前后中医症状积分及血清卵泡刺激素(FSH)、黄体生成素(LH)、雌二醇(E2)、FSH/LH值的变化,统计2组临床疗效及不良反应发生情况。结果治疗后,治疗组月经停闭时间、经色黯或色紫有块、腰酸膝软、头晕耳鸣、经期小腹刺痛、面色及唇甲色泽、潮热汗出、烦躁易怒、倦怠乏力积分及总积分均明显降低(P均<0.05),对照组仅月经停闭时间、潮热汗出积分和总积分均明显降低(P均<0.05),治疗后治疗组经色黯或色紫有块、腰酸膝软、头晕耳鸣、经期小腹刺痛积分和总积分均明显低于对照组(P均<0.05)。治疗后2组患者血清FSH、LH水平及FSH/LH均明显降低(P均<0.05),血清E2水平均明显增高(P均<0.05),其中治疗组血清FSH水平明显低于对照组(P<0.05)。2组总体疗效比较差异无统计学意义(P>0.05),2组治疗期间均未见明显不良反应。结论补肾活血方较雌孕激素序贯疗法能更有效缓解肾虚血瘀型早发性卵巢功能不全患者临床症状,调节FSH作用更强,促进卵巢功能恢复效果更好,且安全性高。
Objective It is to observe the clinical efficacy of Bushen Huoxue decoction in the patients with premature ovarian insufficiency with type of kidney deficiency and blood stasis. Methods Forty patients with premature ovarian insufficiency of type of kidney deficiency and blood stasis were divided into treatment group and control group,with 20 cases in each group.The treatment group was treated with Bushen Huoxue decoction by oral administration, and the control group was treated with hormone replacement therapy with estradiol valerate and progesterone. Both groups were treated for 3 months. The changes of TCM symptom scores and serum follicle stimulating hormone (FSH), luteinizing hormone (LH), estradiol (E 2) and FSH/LH values were observed before and after treatment. The clinical efficacy and adverse reactions of the two groups were statistically analyzed. Results After treatment, the scores of menstrual closure time, dark or purple menstruation, tired waist and knees, dizziness-tinnitus, menstrual lower abdominal pain, complexion and lip color, hot flashes, irritability, fatigue and total scores were significantly decreased in the treatment group( P <0.05), but only the scores of menstrual closure time, hot flashes and total scores were significantly decreased in the control group ( P <0.05), the scores of dark or purple menstruation, tired waist and knees, dizziness-tinnitus, menstrual lower abdominal pain, and total scores were significantly in the treatment group were lower than the control group ( P <0.05). After treatment, the levels of serum FSH, LH and FSH/LH in the two groups were significantly decreased ( P <0.05), and serum E 2 levels were significantly increased ( P <0.05), the serum FSH level in the treatment group was significantly lower than that in the control group ( P <0.05).The overall efficacy of the treatment group was significantly better than that of the control group ( P <0.05), and no significant adverse reactions were observed during the treatment.
