摘要
目的探讨经外周静脉穿刺置入中心静脉导管(PICC)团队在降低极低出生体质量(VLBW)早产儿PICC置管后并发机械性静脉炎中的作用。方法采用目的抽样法选取2017年9月—2018年4月住院期间留置PICC导管、采用常规流程置管和维护的VLBW早产儿70例为对照组。成立PICC团队,回顾分析对照组VLBW早产儿PICC置管后并发机械性静脉炎的影响因素,制订改进措施并实施,选取2018年5月—12月建立PICC团队后留置PICC导管的VLBW早产儿70例为观察组,比较2组患儿一次性置管成功率、操作时间、机械性静脉炎发生率的情况。结果观察组患儿一次性穿刺成功率高于对照组(χ^2=5.833,P=0.016),操作时间短于对照组(U=-2.347,P=0.019),机械性静脉炎发生率低于对照组(χ^2=4.516,P=0.034)。结论 PICC团队的建立有效提高了VLBW早产儿PICC置管的效果和维护质量,保证患儿的导管使用安全。
Objective To investigate the effect of PICC team in prevention of peripherally inserted central catheter-related(PICC-related)mechanical phlebitis among very-low-birth-weight(VLBW)premature infants. Methods Purposive sampling methods were used to recruit 70 hospitalized VLBW premature infants with PICC between September 2017 and April 2018. A PICC team was set up to analyze the influence factors of PICC-related mechanical phlebitis, and to carry out countermeasures. Another 70 hospitalized VLBW premature infants with PICC between May 2018 and December 2018 were included as the observation group. The one-time success rate of puncture, time cost of catheterization, incidence of mechanical phlebitis were observed and compared between two groups. Results The one-time success rate of puncture in the observation group was higher than that in the control group(χ^2=5.833, P=0.016). The time cost of catheterization was shorter in the observation group than that in the control group(U=-2.347,P=0.019). The incidence of mechanical phlebitis in the observation group was lower than that in the control group(χ^2=4.516, P=0.034). Conclusion The establishment of PICC team can effectively improve the quality of PICC maintenance and ensure the safety of PICC insertion in VLBW premature infants.
