摘要
目的探讨维持性血液透析患者不宁腿综合征的临床特点及相关因素。方法97例维持性血液透析患者,依据有无合并不宁腿综合征分为合并不宁腿综合征组(12例)和无合并不宁腿综合征组(85例)。对维持性血液透析患者不宁腿综合征临床特点与相关因素进行分析。结果 97例维持性血液透析患者中,不宁腿综合征发生率为12.37%(12/97)。合并不宁腿综合征组的血磷水平(1.82±0.58)mmol/L、全段甲状旁腺素(iPTH)水平(302.50±6.50)pg/ml高于无合并不宁腿综合征组的(1.55±0.35)mmol/L、(234.60±8.50)pg/ml,差异有统计学意义(P<0.05);合并不宁腿综合征组的锻炼状况差于无合并不宁腿综合征组,差异有统计学意义(P<0.05)。维持性血液透析患者的不宁腿综合征多因素Logistic回归分析显示:血磷、iPTH水平高、运动锻炼少为维持性血液透析患者合并不宁腿综合征的危险因素(P<0.05)。结论机体高磷、缺少运动、高iPTH等因素均与维持性血液透析患者不宁腿综合征的发生相关,临床需加强重视,以降低不宁腿综合征发生率,提高维持性血液透析患者的透析效果,延长生存时间。
Objective To discuss the clinical characteristics and related factors of restless legs syndrome in maintenance hemodialysis patients. Methods A total of 97 maintenance hemodialysis patients were divided by presence or absence of restless legs syndrome into restless legs syndrome group(12 cases) and non-restless legs syndrome group(85 cases). The clinical characteristics and related factors of restless legs syndrome in maintenance hemodialysis patients was analyzed. Results The incidence of restless legs syndrome was 12.37%(12/97) in 97 patients with maintenance hemodialysis. Restless legs syndrome group had higher blood phosphorus level as(1.82±0.58) mmol/L and intact parathyroid hormone(iPTH) as(302.50±6.50) pg/ml than(1.55±0.35) mmol/L and(234.60±8.50) pg/ml in non-restless legs syndrome group, and the difference was statistically significant(P<0.05). Restless legs syndrome group had worse exercise condition than non-restless legs syndrome group, and the difference was statistically significant(P<0.05). Logistic regression analysis showed that elevated levels of blood phosphorus and iPTH and less exercise were risk factors for restless legs syndrome of maintenance hemodialysis patients(P<0.05). Conclusion High phosphorus, lack of exercise and high iPTH are all related to restless legs syndrome in maintenance hemodialysis patients. Clinical attention should be paid to reduce the incidence of restless legs syndrome, improve the dialysis effect of maintenance hemodialysis patients and prolong survival time.
