摘要
目的:探究补肾活血汤联合电针及经椎旁肌间隙入路固定治疗胸腰椎爆裂性骨折的临床疗效。方法:98例胸腰椎爆裂性骨折患者作为研究对象,根据治疗方法不同意随机数字表法分为研究组及对照组,对照组(49例)经椎旁肌间隙入路固定治疗,研究组(49例)则采取补肾活血汤联合电针及经椎旁肌间隙入路固定治疗,比较两组患者疗效、术后7 d患者疼痛评分(视觉模拟疼痛评分,VAS)及术后3个月患者Oswestry功能障碍指数(ODI)评分、术前及术后3个月患者Cobb角及伤椎前缘高度、骨折愈合评分系统(RUSS)评分及骨折愈合时间、术后并发症情况及患者治疗满意率。结果:研究组患者有效率(97.96%)高于对照组(85.71%),P<0.05;术后3个月研究组患者ODI评分及VAS评分均优于对照组,P<0.01;术前两组患者Cobb角及伤椎前缘高度相当,P>0.05,术后3个月研究组患者Cobb角及伤椎前缘高度优于对照组,P<0.05;研究组患者RUSS评分优于对照组,骨折愈合时间早于对照组,P<0.05;研究组并发症率(2.04%)低于对照组(14.29%),治疗满意率(93.88%)高于对照组(77.55%),P<0.05,差异具有统计学意义。结论:补肾活血汤联合电针及经椎旁肌间隙入路固定治疗胸腰椎爆裂性骨折的临床疗效显著,患者骨折愈合好,胸腰椎功能恢复好,且患者术后疼痛轻,并发症率低,安全性高,患者满意率高,值得临床推广以及应用。
Objective:To explore the clinical effect of Bushen Huoxue Decoction combined with electroacupuncture and transpedicular interstitial approach in the treatment of thoracolumbar burst fracture.Methods:98 cases of thoracolumbar burst fracture as the research objects, according to the treatment and random number table method,were divided into the research group and the control group. The control group(49 cases) used transpedicular interstitial approach treatment and the research group(49 cases) were treated with Bushen Huoxue Decoctioncombined with the transpedicular interstitial approach treatment. We compared the curative effect of two groups, pain score(visual analog pain score, VAS)7 d aftertreatment and, Oswestry functional disorder index(ODI) score,Cobb Angle and anterior edge height of injured vertebrae before and 3 months after surgery, fracture healing scoring system(RUSS) score, fracture healing time, postoperative complications and treatment satisfaction rate of patients. Results:The effective rate(97.96%) of the study group was higher than that of the control group(85.71%)(P<0.05). The ODI score and VAS score of the study group were better than those of the control group 3 months after surgery(P<0.01). The Cobb Angle and the anterior edge of the injured vertebra were the same in the two groups before surgery(P>0.05). RUSS score of the study group was better than that of the control group, and fracture healing time was earlier than that of the control group,(P<0.05). The complication rate of the study group(2.04%) was lower than that of the control group(14.29%).The satisfaction rate of the study group(93.88%) was higher than that of the control group(77.55%), and the difference was statistically significant(P<0.05).Conclusion:Bushen Huoxue Decoction combined with electroacupuncture and transpedicular interstitial fixation for the treatment of thoracolumbar burst fracture has a significant clinical effect. The fracture healing of the patients is good, and the thoracolumbar function is well restored.
