摘要
目的探讨白介素4(IL-4)内含子3基因多态性与T2DM周围神经病变(DPN)的关系。方法选取108例T2DM患者(T2DM组)、69例DPN患者(DPN组)及50名健康体检人员(NC组),分析各组IL-4内含子3基因多态性表达差异。结果与NC组比较,T2DM、DPN组TG、LDL-C、HbA1c、FPG、IL-4、免疫球蛋白E(IgE)、TNF-α升高,HDL-C、正中神经运动神经传导速度(MMCV)、腓总神经运动神经传导速度(PMCV)、正中神经感觉神经传导速度(MSCV)及腓总神经感觉神经传导速度(PSCV)降低(P<0.05或P<0.01)。与T2DM组比较,DPN组病程、IL-4、IgE、TNF-α升高,MMCV、PMCV、MSCV及PSCV降低(P<0.05或P<0.01)。P2P2型患者IL-4高于P1P2型、P1P1型患者,而MSCV及PSCV低于P1P2型、P1P1型患者(P<0.05)。Logistic回归分析结果显示,糖尿病病程、LDL-C、TNF-α、IL-4、IgE、MMCV是DPN的危险因素(P<0.05或P<0.01)。结论IL-4内含子3基因多态性为DPN发病的风险因素,可能通过影响IL-4、IgE表达发挥作用。
Objective To investigate the relationship between interleukin-4(IL-4)intron 3 gene polymorphism and peripheral neuropathy in type 2 diabetic patients.Methods A total of 227 subjects were enrolled in this study and divided into three groups:patients with T2 DM(T2 DM group,n=108),patients with diabetic peripheral neuropathy(DPN group,n=69)and healthy volunteers(NC group,n=50). The differences in the polymorphism of interleukin 4 intron 3 were analyzed in all the three groups.Results TG,LDL-C,HbA1 c,FPG,IL-4,IgE and TNF-α were higher,HDL-C,MMCV,PMCV,MSCV and PSCV were lower in T2 DM and DPN group than in NC group(P<0. 05 or P<0. 01). Diabetes duration,IL-4,IgE and TNF-α were higher,MMCV、PMCV、MSCV and PSCV were lower in DPN group than in T2 DM group(P<0. 05 or P<0. 01). IL-4 was higher,MSCV and PSCV were lower in P2 P2 genotype than in P1 P2 and P1 P1 genotypes. Logistic regression analysis showed that diabetes duration,LDL-C,TNF-α、IL-4,IgE,MMCV were risk factors for DPN(P<0. 05 or P<0. 01).Conclusion IL-4 intron 3 polymorphism is a risk factor for the pathogenesis of DPN,probably by influencing IL-4,IgE expression.
