CEREC数字化椅旁修复与传统热压铸技术在全瓷髓腔固位冠修复中的临床效果比较 被引量:14

Comparison of clinical effects between CEREC digital chairside restorative system and traditional heat-pressed technique for lithium disilicate endocrown restoration of posterior teeth

摘要 目的:比较分析CEREC数字化椅旁修复与传统热压铸技术对根管治疗术后全瓷髓腔固位冠修复的临床效果.方法:选择西安交通大学修复科就诊行二硅酸锂陶瓷髓腔固位冠修复患者56例,共62颗修复体,随机分成CEREC数字化椅旁修复组(29例患者,32颗修复体)与传统热压铸瓷修复组(27例患者,30颗修复体).两组所有病例均为行完善根管治疗后的后牙,记录每例修复体制取印模时间.修复完成后1个月后对两组患者进行修复后满意度调查.并于修复1年后,参考改良型美国公共健康服务标准(modified USPHS),对修复体边缘适合性、边缘染色、修复体折裂/脱落、修复体外形、患牙继发龋、牙龈健康情况及牙折进行评价.结果:CEREC数字化椅旁修复组制取数字化印模时间[(4.55±1.07) min]明显少于传统热压铸修复组[(12.51±0.30) min](P<0.05).患者对CEREC数字化椅旁修复技术整体就诊满意度为96.6%,高于传统热压铸修复组(P<0.05).两组修复体效果USPHS各项评价指标比较无显著差异(P>0.05).结论:CEREC数字化椅旁修复系统应用于根管治疗术后牙体缺损的全瓷髓腔固位冠修复临床效果满意,极大缩短临床治疗时间,提高患者就诊满意度,值得临床推广. Objective:To compare and analyze the clinical effects of CEREC digital chairside restorative system and traditional heat-pressed casting technique on lithium disilicate endocrown restoring the posterior teeth defect after root canal therapy.Methods:Fifty-six patients with 62 lithium disilicate endocrown restorations were selected from the department of prosthodontics,Stomatology Hospital of Xi’an Jiaotong University,and divided into CEREC digital chairside restorative system group(29 cases,32 endocrowns)and traditional heat-pressed ceramic restoration group(27 cases,30 endocrowns)randomly.The taking impression time of each restoration was recorded.The satisfaction survey was investigated 1 month after restoration.Also,the patients were followed up 1 year after restoration and the restorations were assessed using modified USPHS criteria including marginal integrity,marginal discoloration,restoration fracture/debond,secondary caries,gingivitis,tooth fracture and food impaction.Results:The time of taking digital impression of CEREC digital chairside restorative group(4.55±1.07 mins)was shorter than that in traditional heat-pressed casting restorative group(12.51±0.30 mins,P<0.05)significantly.The patients satisfaction with CEREC digital chairside restorative technique was 96.6%,which was higher than that of the traditional heat-pressed restorative group obviously(P<0.05).There was no significant difference of restoration evaluation using USPHS criteria between the two groups(P>0.05).Conclusion:Lithium disilicate endocrown fabricated with CEREC digital chair-side restorative system can achieve satisfactory clinical results in restoring posterior teeth defect after root canal therapy,shorten the clinical treatment time greatly,and improve the patient’s satisfaction.
