摘要
目的探讨出院电话随访式延续护理在脑卒中患者自我管理中的应用效果。方法选取南阳市第一人民医院符合纳入标准的160例出院脑卒中患者,根据出院日期单双号分为观察组和对照组,每组各80例。对照组给予出院和复诊时常规康复指导,观察组在此基础上定期电话随访;采用脑卒中自我管理行为量表评价两组患者出院时、出院后1个月、2个月、3个月时自我管理能力。结果两组患者自我管理能力评分差异有统计学意义(F组间=5.65,P<0.05);组内各时间点评分差异有统计学意义(F时间=1733.86,P<0.01),干预和时间存在交互作用(F交互=89.59,P<0.01),随着出院时间延长,观察组评分上升幅度较对照组明显;出院时、出院后1个月,两组评分差异无统计学意义(F值分别为0.06和0.161,P>0.05),出院后2个月、3个月时差异有统计学意义(F值分别为12.76和28.54,P<0.01)。结论电话随访简便易行,可强化出院脑卒中患者自我管理意识,改善自我管理能力,促进患者康复。
Objective To explore the application effect of continuity of care based on the telephone follow-up in self-management of discharged patients with stroke.Methods A total of 160 discharged patients with stroke who met inclusion criteria were selected as study subjects.They were divided into observation group(n=80,the regular telephone follow-up was added on the base of routine rehabilitation guidance)and control group(n=80,routine rehabilitation guidance at discharge and when returning visit)according to the discharge order.The self-management ability of two groups was evaluated with the self-management behavior scale for stroke patients at discharge,1 month,2 months and 3 months after discharge.Results The difference of self-management ability were statistically significant between two groups(Fgroupvalue was 5.65,P<0.05).There were statistical significance between scores of two competencies in a group at different time points(F timevalue was 1733.86,P<0.01).There was an interaction between time and intervention (F time*groupvalue was 89.59,P<0.01).Along with the prolonging time of discharge,the increase scores of the observation group was more obvious than that of the control group.There were no statistically significant difference between scores of two competencies at discharge and 1 month after discharge separately(F values were 0.06 and 0.161 respectively,P>0.05),but there were statistically significant difference between 2 months and 3 months after discharge separately(F values were 12.76 and 28.54 respectively,P<0.01). Conclusion The telephone follow-up for patients with stroke is simple and convenient,which can strengthen the self-management consciousness,improve the self-management ability,and promote the rehabilitation of patients.
