摘要
目的 研究益生菌应用对2型糖尿病患者肠道菌群的影响和临床应用价值。方法 选取2型糖尿病患者给予口服益生菌24周,收集受试者基线状态和终点时粪便,采用16srRNA高通道测序鉴定和分析各样本肠道菌群,比较干预前后血糖、血脂、胰岛素抵抗的变化。结果共有75例患者完成临床观察,16srRNA高通道测序显示终点时受试者放线菌门和软壁菌门增多者比例增加(分别为37.8%和75.7%),属水平分析显示肠道益生菌数量增多者和条件致病菌数量减少者的例数均增加。按干预前后分组行聚类分析显示同组样本的肠道菌群具有较高的相似性。与基线状态比较,干预后终点状态受试者空腹血糖显著降低(P<0.05),甘油三酯和胆固醇达标比例增加,HOMA-IR显著改善(P<0.05)。结论益生菌应用能够调节2型糖尿病患者的肠道菌群,促进空腹血糖达标和改善胰岛素抵抗,有利于改善血脂代谢。
Objective To study the effects of probiotics on gut microbiota in type2diabetes patients and evaluate their therapy value.Methods The feces of patients with type2diabetes were collected at baseline and end points after24 weeks of oral probiotics administration.The gut microbiota were identified and analyzed by16srRNA high throughput sequencing.The fasting glucose,serum lipid,HOMA-IR indexes were compared before and after the intervention.Results A total of 75 patients completed the clinical investigation.Compared with the baseline,fasting glucose decreased significantly ( P < 0.05),HOMA-IR improved significantly ( P < 0.05),the proportion of patients whose triglyceride and cholesterol concentration reaching the therapy goal was increased in the end point.The16srRNA high throughput sequencing of gut microbiota showed the proportion of Actinobacteria and Tenericutes of the post group increased (37.8 % and 75.7 % respectively) in the end point.Genus level analysis showed that the number of intestinal probiotics increased and the number of conditional pathogens microbiota decreased.Cluster analysis showed that before and after intervention the gut microbiota of the each group had high similarity.Conclusion Probiotics can regulate gut microbiota in type2diabetes mellitus,improve fasting glucose control and insulin resistance,and ameliorate lipid metabolism.
