玻璃体内注射阿柏西普与雷珠单抗治疗黄斑变性的效果比较 被引量:12

Comparison of the efficacy between intravitreal injection of aflibercept and ranibizumab for the treatment of macular degeneration

摘要 目的比较玻璃体内注射阿柏西普与雷珠单抗治疗湿性年龄相关性黄斑变性(AMD)的短期效果。方法对2018年6月至12月湿性AMD 60例(60只眼)的临床资料进行回顾性研究。患者随机分为阿柏西普组及雷珠单抗组,每组各30只眼。随访12周。比较两组的效果。结果治疗前患者基线资料两组间差异均无统计学意义(P>0.05),治疗后4周及12周两组的视力(BCVA)、黄斑中心区厚度(CMT)、脉络膜新生血管(CNV)渗漏程度及多焦ERG的改变均较术前好转,差异均有统计学意义(P<0.05)。各随访时间点,两组之间最佳矫正视力、视网膜渗漏情况及多焦ERG改变情况比较,差异均无统计意义(P>0.05),而各时间点阿柏西普组CMT均低于雷珠单抗组,差异均有统计学意义(P<0.05)。结论阿柏西普与雷珠单抗治疗湿性AMD在改善视力、CNV渗漏程度及多焦ERG情况效果相当,但阿柏西普对黄斑水肿的降低作用更为明显。 Objective To compare the short-term efficacy between intravitreal injection of aflibercept and ranibizumab for the treatment of wet age-related macular degeneration (wAMD). Methods Clinical retrospective study. Total of 60 eyes of 60 cases with wAMD from Jun.2018 to Dec.2018 were randomly divided into aflibercept group and ranibizumab group, with 30 eyes each.The follow-up time was 12 weeks.The efficacy of the two groups were compared. Results There was no significant difference in baseline data between the two groups before treatment (P>0.05). The best corrected visual acuity (BCVA), central macular thickness (CMT) and choroidal neovacularization (CNV) leakage and multi-focal ERG in the two groups at 4 weeks and 12 weeks after treatment were improved, and the differences were statistically significant (P<0.05). At each time point of follw-up, there was no statistically significant difference in the best corrected visual acuity, retinal leakage or the changes of multi-focal ERG between the two groups (P>0.05). The CMT of the aflibercept group were lower than that of the ranibizumab group, and the difference was statistically significant (P<0.05). Conclusion Intravitreal injection of aflibercept and ranibizumab for the treatment of wAMD are equally effective in improving visual acuity, CNV leakage and multi-focal ERG, but aflibercept has a more significant effect on the reduction of macular edema.
