摘要
目的探讨切开复位内固定术(ORIF)联合韧带修复治疗踝关节骨折合并三角韧带Ⅱ度损伤的临床效果。方法选取我院收治的44例踝关节骨折合并三角韧带Ⅱ度损伤患者为研究对象,根据治疗方式的不同将其分为对照组(22例,ORIF)与观察组(22例,ORIF+带线锚钉韧带修复)。比较两组患者的治疗效果。结果术后6个月,观察组患者的治疗总有效率显著高于对照组(P<0.05);术后6个月,两组患者的VAS评分、TT、内踝间隙及ATT均降低,AOFAS评分均升高,且观察组均优于对照组(P<0.05);术后1个月,两组患者的BALP、BGP、PICP、PINP、OPG水平均升高,且观察组高于对照组(P<0.05);术后6个月,两组患者的SF-36各项评分均升高,且观察组高于对照组(P<0.05)。结论ORIF联合韧带修复治疗踝关节骨折合并三角韧带Ⅱ度损伤患者的效果显著,值得临床推广应用。
Objective To investigate the clinical effect of open reduction and internal fixation (ORIF) combined with ligament repair in patients with ankle fracture combined with grade Ⅱ injury of deltaid ligament. Methods A total of 44 patients with ankle fracture combined with grade Ⅱ injury of deltaid ligament were selected as the study objects. According to the different treatment methods, the patients were divided into control group(22 cases, ORIF) and observation group (22 cases, ORIF+ligament repairment with suture anchor). The treatment effects between two groups were compared. Results At six months after operation, the total effective rate of treatment in the observation group was significantly higher than that in the control group (P<0.05);at 6 months after operation, the VAS scores, TT, medial malleolus space and ATT of the two groups decreased, while AOFAS scores increased, and those in the observation group were better than the control group (P<0.05);at 1 month after operation, the levels of BALP, BGP, PICP, PINP and OPG of the two groups increased, and those in the observation group were higher than the control group (P<0.05);at 6 months after operation, the SF-36 scores of the two groups increased, and those in the observation group were higher than the control group(P<0.05). Conclusion ORIF combined with ligament repair in the treatment of ankle fracture with grade Ⅱ injury of deltaid ligament has a significant effect, and it is worthy of clinical promotion and application.
