O-MAR重建在去除颅内动脉瘤弹簧圈栓塞术后金属伪影的价值 被引量:3

Clinical evaluation of orthopedic metal artifact reduction(O-MAR) in reducing metallic artifacts in CT of intracranial aneurysms after endovascular coil treatment

摘要 目的探讨O-MAR算法重建在去除颅内动脉瘤弹簧圈栓塞术后金属伪影中的效能。方法连续入选颅内动脉瘤弹簧圈栓塞术后行CT血管成像(CTA)复查的患者28例,19例行颈脑动脉CTA扫描,9例单纯脑动脉CTA扫描,分别获取颅底至颅顶层面常规迭代重建图像及O-MAR算法重建图像。将常规迭代重建图像设置为A组,O-MAR算法重建图像设置为B组。由1名影像诊断医师对2组图像瘤体中间层面伪影明显区,随机选取3处进行CT值标准方差(SD)值测量,取平均值,行配对t检验;由2名影像诊断医师依据弹簧圈本身形态及周围软组织的可识别度,独立对2组图像进行主观评分(4分法)。结果A组平均SD值(108±40),远高于B组的(43±19),2组图像SD差异具有统计学意义(P<0.01);图像质量主观评分:A组(0.8±0.7)分,B组(2.3±0.8)分,两者相比差异具有统计学意义(P<0.01)。结论O-MAR算法重建能有效去除颅内动脉瘤栓塞术后金属伪影。 Objective To investigate the artifacts reducing effect of orthopedic metal artifact reduction(O-MAR,Philips Healthcare)algorithm for patients with intracranial aneurysms after endovascular coiling.Methods Twenty-eight patients after endovascular treatment of intracranial aneurysms(16 cases stent-assisted coiling,12 cases standard coiling)undergoing brain or carotid-brain CT angiography were included in this study,Non-OMAR images were reconstructed with iterative reconstruction alone,and O-MAR images were reconstructed with the iterative reconstruction plus O-MAR algorithms.The system reconstructed the non O-MAR processed images(group A)in addition to the OMAR processed images(group B).Image noise around metallic coils were compared between the O-MAR and non-OMAR images.Quantitatively,the mean SD in three ROIs on axial images were compared between the O-MAR and non-OMAR images.Qualitatively,two blinded,independent radiologists visually evaluated the metallic coil artefacts utilizing a four-point scale.Results The image noise on adjacent tissues of coiling on images was significantly lower in O-MAR datasets compared with those of images without O-MAR(43±19 vs 108±40,P<0.01).Better visual scores were obtained with the O-MAR algorithm(0.8±0.7 vs 2.3±0.8,P<0.01).Conclusion The O-MAR algorithm can significantly reduce artefacts caused by metallic coils of patients with intracranial aneurysms after endovascular treatment compared with the non-OMAR algorithm.This technique can potentially increase CT performance for the evaluation of post-coil embolization complications.
