摘要
目的:对上海市中老年2型糖尿病(T2DM)患者健康体适能、骨密度(BMD)及血管硬度进行测量和分析,提高以运动手段治疗T2DM方法的针对性。方法:以120例50~70岁T2DM患者为对象,测试其身高、体质量、BMI、BMD等指标,分析中老年T2DM患者健康体适能、BMD、血管硬度的相关性。结果:①男性患者在肺活量、最大摄氧量、BMD、体质量、体脂率和握力等指标上均优于女性患者(P<0.01)。②男性患者病程与其BMD和左侧踝臂血压指数(Labi)呈负相关(P<0.05);女性患者年龄与其左右侧踝臂血压指数(abi)呈负相关(P<0.05)。③患者BMD与体质量、BMI、握力和肺活量等健康体适能指标呈显著性正相关;血管硬度与肺活量和最大摄氧量指标间呈一定的负相关,但不具有显著性。④握力水平大于24N的患者体质量、体脂率、肺活量、最大摄氧量和BMD等指标均明显优于握力小于24N的患者(P<0.01)。结论:男性患者多数健康体适能优于女性;年龄和病程的增长是影响患者BMD和踝臂脉搏波传导速度(baPWV)等的重要因素,且会因性别而不同;通过增强肌肉力量和心肺功能的练习可降低体脂含量,提高骨骼矿物质含量并改善患者健康体适能。
Objective: To measure the treatment means on T2DM and explore the correlation among health-related physical fitness,bone mineral density(BMD),and vascular hardness on elderly patients with T2DM of Shanghai city. Methods: 120 patients with T2DM,age between 50~70 years old were enrolled in this study.Height,body weight,BMI,BMD were measured,analyzed correlation among health-related physical fitness,BMD and vascular stiffness. Results:① Male patients with T2DM were better than female patients ( P <0.01) on vital capacity,maximum oxygen uptake,BMD,body weight,body fat percentage and grip strength.② The duration of male patients was negatively correlated with their BMD and Labi ( P <0.05).The age of female patients was negatively correlated with the left and right abi ( P <0.05).③ There was a significant positive correlation between BMD and body weight,BMI,grip strength and vital capacity;a negative correlation between vascular hardness and index of vital capacity and maximum oxygen uptake,but without significance.④ The indexes like body weight,body fat percentage,vital capacity,maximum oxygen uptake and BMD of patients whose grip strength>24N were significantly higher in those with grip strength <24N ( P < 0.01 ). Conclusions: The health fitness of most T2DM males is better than that of females.The patients of T2DM,BMD and baPWV were influenced by the growth of the age and course of the disease,and the gender was also one of the important factors.Enhancing muscle strength and cardiopulmonary function exercise can reduce body fat and the bone mineral content,improve the health-related physical fitness of patients.
