复合保温措施用于神经外科颅脑手术患者体温及预后的影响观察 被引量:3

Effect of compound insulation measures on body temperature and prognosis of patients undergoing neurosurgical craniocerebral surgery

摘要 目的观察复合保温措施用于神经外科颅脑手术患者体温及预后的影响。方法选取2017年10月至2018年10月就诊本院神经外科的颅脑手术患者60例作为研究对象,采用随机数字表法分为两组,各30例。观察组采用复合保温措施,对照组采用常规保温方式,对比两组患者围术期体温变化及神经功能恢复情况。结果观察组术中开颅时、手术结束时、术后苏醒时体温分别为(33.89±0.20)℃、(34.02±0.11)℃、(36.19±0.20)℃,均高于对照组,差异均有统计学意义(均P<0.05);观察组非计划性低体温、寒战、术后感染发生率分别为13.33%、3.33%、10.00%,均低于对照组,差异均有统计学意义(均P<0.05);观察组术后拔管时间(10.22±5.31)h,低于对照组,差异有统计学意义(P<0.05);观察组术后神经功能恢复情况显著优于对照组,差异有统计学意义(P<0.05)。结论复合保温措施适用于神经外科颅脑手术患者,在保障患者亚低温治疗有效性的前提下,能够降低患者术中体温过低,寒战发生风险,而且术后拔管时间减少,降低患者术后感染风险,患者术后神经功能恢复程度更优,有助于患者预后康复效果的提升。 Objective To observe the effect of compound insulation measures on body temperature and prognosis of patients undergoing neurosurgical craniocerebral surgery. Methods 60 patients undergoing neurosurgical craniocerebral surgery at our hospital from October, 2017 to October, 2018 were selected as the objects of tooth decay, and were divided into an observation group and a control group by using the random digital method. The composite heat preservation measures were used in the observation group. In the control group, routine thermal insulation was used. The changes of body temperature and the recovery of the neurological function were compared between the two groups. Results The body temperatures when the skull was opened, the operations was over, and the patients came to were (33.89±0.20)℃,(34.02±0.11)℃, and (36.19±0.20)℃ in the observation group, which were higher than those in the control group (all P < 0.05). The incidences of unplanned hypothermia, shivering, and postoperative infection were 13.33%, 3.33%, and 10.00% in the observation group, which were lower than those in the control group (all P<0.05). The extubation time after the operation was (10.22 ±5.31) hours in the observation group, which was shoter than that in the control group (P < 0.05). The recovery of nerve function was significantly better in the observation group than in the control group (P < 0.05). Conclusion The composite thermal insulation measures are suitable for patients taking neurosurgical craniocerebral surgery, and can reduce the risk of low body temperature and shivering during operation as well as postoperative infection, shorten postoperative extubation time, promote the patients’ recovery of neurological function after operation and prognosis.
