摘要
目的探讨低分子肝素(LMWH)结合主动免疫对复发性自然流产(RSA)患者的临床治疗效果及对患者血液高凝状态的改善效果。方法选取2017年6月~2018年7月于我院就诊的68例RSA患者作为研究对象,通过随机数字表法分为观察组和对照组,每组各34例。对照组给予主动免疫疗法,确定妊娠后联合常规保胎治疗。观察组在此基础上,联合LMWH治疗。比较两组治疗后的保胎成功率、血清D-二聚体(D-D)水平、凝血酶时间(TT)和血流动力学变化。结果治疗后,观察组足月产率、早产胎儿存活率和保胎成功率均高于对照组,而早产胎儿死亡率、流产率低于对照组,差异有统计学意义(P<0.05)。观察组血清D-D水平低于对照组,而TT长于对照组,差异有统计学意义(P<0.05)。治疗后,观察组全血高切粘度、全血低切粘度和血浆粘度均低于对照组,差异有统计学意义(P<0.05)。结论LMWH结合主动免疫治疗RSA,能显著提升保胎成功率,改善患者血液动力学,值得临床推广应用。
Objective To investigate the effect of Low Molecular Weight Heparin (LMWH) combined with active immunization on patients with recurrent spontaneous abortion (RSA) and its influences on hemodynamics. Methods A total of 68 RSA patients in our hospital treated from June 2017 to July 2018 were selected. They were divided into the observation group (34 cases) and the control group (34 cases) according to random number table method. The control group was treated by the active immunotherapy, treating them by the usual preventing miscarriage theory after being pregnant. Based on that, the observation group was treated by LMWH. The success rate of preventing miscarriage, the levels of D-dimer (D-D), thrombin time (TT) and hemodynamics were observed between two groups. Results The rate for term infant, live premature baby, succeed tocolysis in observation group were higher than those in control group(P<0.05), the proportions of died premature and aborted gravida were lower than those in control group (P<0.05). The level of D-D in the observation group was lower than that in the control group (P<0.05), TT in the observation group was higher than that in the control group (P<0.05), the blood viscosity, whole blood low shear viscosity and plasma viscosity in the observation group were lower than those in the control group(P<0.05). Conclusion LMWH combined with active immunization has a significant effect on RSA, it can improve patients′ hemodynamics.
