摘要
目的观察腹腔镜下筋膜内与筋膜间保留神经的前列腺癌根治术对患者围术期指标、勃起功能、控尿功能及术后并发症的影响。方法我院行腹腔镜下筋膜内与筋膜间保留神经的前列腺癌根治术患者118例,按手术方法分为筋膜内组(n=56)和筋膜间组(n=62)。比较两组围术期指标及术后勃起功能、控尿功能评估结果,观察术后并发症发生情况。结果两组围手术期各指标、术后并发症发生率比较,差异无统计学意义(P>0. 05)。术后3、6月时,筋膜内组勃起功能正常率高于筋膜间组,控尿功能优于筋膜间组(P<0. 05)。结论腹腔镜下筋膜内与筋膜间保留神经的前列腺癌根治术治疗局限性前列腺癌手术情况和术后并发症发生情况相近,但行筋膜内手术患者术后勃起功能、控尿功能恢复情况优于筋膜间手术。
Objective To observe the effects of radical prostatectomy of laparoscopic intrafascial and interfascial nerve preservation on perioperative indexes,erectile function,urinary control function and postoperative complications.Methods One hundred and eighteen patients undergoing radical prostatectomy of laparoscopic intrafascial and interfascial nerve preservation in our hospital were divided into intrafascial group(n = 56) and interfascial group(n = 62) according to surgical methods.The perioperative indexes,postoperative erectile function and urinary control function assessment results were compared between the two groups and the occurrence of postoperative complications were observed.Results There was no significant difference in the perioperative indexes and the incidence rates of postoperative complications between the two groups(P>0. 05).After 3 and 6 months of operation,the normal rate of erectile function was significantly higher and the urinary control function was better in the intrafascial group than those in the interfascial group(P <0. 05).Conclusion Radical prostatectomy of laparoscopic intrafascial and interfascial nerve preservation has similar surgical conditions and postoperative complications in patients with localized prostate cancer.However,patients with intrafascial surgery have better recovery of postoperative erectile function and urinary control function than patients with interfascial surgery.
