摘要
目的探讨小儿肺炎支原体肺炎(MPP)血清急性时相反应蛋白水平变化情况及其与疾病严重程度的相关性。方法回顾性分析124例MPP患儿临床资料,分别比较不同住院时间、累及肺段范围、肺外脏器受累及伴严重肺内并发症情况下,患儿血清急性时相反应蛋白[C反应蛋白(CRP)、α1-酸性糖蛋白(α1-AGP)、结合珠蛋白(HP)、铜蓝蛋白(CP)]水平差异,分析其水平与疾病严重程度的相关性,通过受试者工作特征(ROC)曲线,评估上述指标描述MPP严重程度的诊断效能。结果不同住院时间MPP患儿血清CRP、α1-AGP、HP、CP水平比较差异均无统计学意义(P>0. 05);不同累及肺段范围患儿血清CRP、α1-AGP、HP、CP水平比较差异均有统计学意义(P<0. 05);伴严重肺内并发症MPP患儿血清CRP、α1-AGP、HP、CP水平高于无严重肺内并发症者;有肺外脏器受累MPP患儿血清CRP、α1-AGP、HP、CP水平高于无肺外脏器受累者(P<0. 05)。MPP患儿血清CRP、α1-AGP、HP、CP水平与累及肺段范围、肺外脏器受累、伴严重肺内并发症情况呈正相关(P<0. 05)。ROC曲线显示,血清CRP、α1-AGP、HP、CP水平评估累及肺段是否超过2/3的曲线下面积(AUC)分别为0. 941、0. 879、0. 930、0. 738,评估有无严重肺内并发症的AUC分别0. 918、0. 746、0. 819、0. 673,评估有无肺外脏器受累的AUC分别为0. 946、0. 905、0. 955、0. 740。结论 MPP患儿血清CRP、α1-AGP、HP、CP水平与疾病严重程度关系较为紧密,可为临床监测病情、评估预后提供指导性证据。
Objective To explore the changes of serum acute phase response protein levels in children patients with mycoplasma pneumoniae pneumonia( MPP) and their correlation with disease severity.Methods The clinical data of 124 children patients with MPP were retrospectively analyzed.The hospital stay,involvement extent of lung segments,extrapulmonary involvement,severe intrapulmonary complications and acute phase response protein such as C-reactive protein( CRP),α1-acid glycoprotein(α1-AGP),haptoglobin( HP),and ceruloplasmin( CP) were compared. The correlation between the protein levels and disease severity was analyzed.The diagnostic efficacy of the above indicators for the severity of MPP was assessed by receiver operating characteristic( ROC)curves.Results There were no significant differences in the serum levels of CRP,α1-AGP,HP and CP among MPP children patients with different hospital stay( P>0. 05).There were statistically significant differences in the serum levels of CRP,α1-AGP,HP and CP in children patients with different involvement extent of lung segments( P<0. 05).The serum levels of CRP,α1-AGP,HP and CP in the MPP children patients with severe intrapulmonary complications were significantly higher than those in the patients without severe intrapulmonary complications( P<0. 05).The serum levels of CRP,α1-AGP,HP and CP in the MPP children patients were positively correlated with involvement extent of lung segments,extrapulmonary involvement and severe intrapulmonary complications( P< 0. 05).ROC curves analysis showed that the area under the curve( AUC) of the serum levels of CRP,α1-AGP,HP and CP in evaluating involvement of lung segments more than 2/3 were 0. 941,0. 879,0. 930 and 0. 738,respectively. The AUC values in evaluating whether there were serious intrapulmonary complications were 0. 918,0. 746,0. 819 and 0. 673,respectively. The AUC values in evaluating whether there was involvement of extrapulmonary organs were 0. 946,0. 905,0. 955 and 0. 740,respectively.Conclusion Serum levels of CRP,α1-A
