核心肌群训练引导康复护理对痉挛型脑瘫患儿运动功能的影响 被引量:34

摘要 目的探讨核心肌群训练引导康复护理对痉挛型脑瘫患儿运动功能的影响,为患儿功能康复提供临床指导。方法选择2017年1月至2018年3月宝鸡市妇幼保健院小儿神经康复科的痉挛型脑瘫患儿70例作为研究对象。按照随机数表法分为对照组和观察组,每组35例。对照组采用常规康复及护理,观察组则采用核心肌群训练引导康复护理。两组患儿每周接受5 d训练,连续康复治疗3个月,所有患儿采用粗大运动功能量表(GMFM)、10 m步行测试(10MWT)和阶梯实验(TST)评估治疗前后的运动情况。结果两组患儿治疗后的GMFM评分、10 m步行测试和阶梯实验结果均好于治疗前,而且治疗后观察组患儿GMFM的D区、E区评分分别为(33.91±2.81)分和(38.35±4.12)分,明显高于对照组的(30.28±3.21)分和(33.46±4.87)分,观察组患儿的10MWT为(31.64±14.78) s,明显低于对照组的(42.28±13.21) s,观察组患儿的TST评估(55.64±14.33) min,明显短于对照组的(70.18±16.21) min,差异均有统计学意义(P<0.05)。结论核心肌群训练引导康复护理可以改善患儿粗大运动功能和行走能力,为脑瘫患儿康复提供了一定的临床指导。 Objective To investigate the efficacy of core muscles training and rehabilitation nursing on motor function of children with spastic cerebral palsy. Methods A total of 70 children with spastic cerebral palsy, who admitted to Department of Pediatric Neurorehabilitation of Baoji Maternal and Child Health Hospital from January 2017 to March 2018, were selected and divided into the control group and the observation group according to the random number table method, with 35 cases in each group. The children in the control group were treated with routine rehabilitation, the children in the observation group were trained on the core muscles training on the basis of routine rehabilitation. All rehabilitation training were 5 days per week for 3 months. Before and after treatment, they were assessed with Gross Motor Function Scale (GMFM), 10-meter walk test (10MWT) and timed stair test (TST). Results The GMFM score, 10MWT and TST results of the two groups were better than those before treatment, and the scores of D and E areas of the GMFM in the observation group were 33.91±2.81 and 38.35±4.12, respectively, which were significantly higher than corresponding 30.28 ± 3.21 and 33.46 ± 4.87 of the control group, and 10MWT in the observation group was (31.64±14.78) s, which was significantly lower than (42.28±13.21) s of the control group;the time of TST decreased obviously from (70.18 ± 16.21) min to (55.64 ± 14.33) min, all differences were statistically significant (all P<0.05). Conclusion Core muscles training and rehabilitation nursing can improve the children's large motor function and walking ability and provide some clinical guidance for the rehabilitation of children with cerebral palsy.
