追踪方法学在院内护理会诊重症患者之护理质控中的应用 被引量:11

摘要 目的探讨分析追踪方法学理论在院内重症护理会诊之护理质控中的应用。方法以2017年1月1日至2017年12月31日某院涉及的疑难重症需要其他科室进行专业指导的87例重症患者为研究对象,根据患者入院编号随机分为对照组(43例)和观察组(44例),对照组实施常规护理会诊流程,基于此观察组结合“追踪方法学”理论对重症患者护理质量进行监督;对比分析2017年全年两组重症患者抢救成功率、“三管”(CA-UTI,CR-BSI和VAP)相关性感染发生率、重症患者质量考核评分、气管插管患者口腔护理合格率以及患者家属对护理满意度;同时对重症患者护理人员能力、心理资本水平以及护护关系进行年度评估。结果 2017年全年观察组重症患者质量考核评分、患者气管插管口腔护理合格率以及患者家属对护理满意度明显高于对照组(P<0. 05);且重症患者护理人员能力、心理资本水平以及护护关系评分均明显高于对照组(P<0. 05)。结论追踪方法学应用于院内护理会诊有助于改善重症护理质量和护护关系,提高患者家属满意度,同时有利于提升护理人员能力和心理资本水平。 Objective To investigate the application of tracer methodology in quality control of intensive nursing consultation. Methods From Jan. 1, 2017 to Dec. 31, 2017, 87 critically ill patients of our hospital requiring specialized guidance from other departments were taken as the study objects, and were randomly divided into the control group(43 patients) and the observation group(44 patients). Usual nursing consultation process was implemented in the control group, base on which, the theory of tracer methodology was conjunctively used in the observation group for monitoring care quality. Comparisons of success rate of rescue, the incidence of ‘CA-UTI, CR-BSI and ventilator associated pneumonia(VAP)’, quality assessment score for severe patients, qualified rate of oral nursing for tracheal intubation and satisfaction of patients’ family members throughout 2017 were made. Nurses’ ability, psychological capital and nurse-nurse relationship were also investigated and compared. Results The quality assessment score for severe patients, qualified rate of oral nursing for tracheal intubation and satisfaction of patients’ family members in the observation group were significantly higher than the control group through 2017(P<0.05); nurses’ ability, psychological capital and nurse-nurse relationship scores were both superior to the control group(P<0.05). Conclusions The application of tracer methodology in hospital nursing consultation may be instrumental in improving critical nursing quality and nurse-nurse relationship, enhancing degree of satisfaction of patients’ family members, as well as raising nurses’ ability and the level of psychological capital.
