摘要
目的总结结外NK/T细胞淋巴瘤在氟代-脱氧葡萄糖(18F-FDG)正电子发射型计算机断层扫描(PET/CT)上的表现,并探讨18F-FDGPET/CT在其分期及治疗方案制定中的价值。方法回顾性分析该院2012年10月至2018年9月33例经病理确诊的结外NK/T细胞淋巴瘤患者的PET/CT图像,将PET/CT分期与CT分期结果比较,探讨SUVmax与肿瘤分期、有无B症状、乳酸脱氢酶、Ki-67之间的关系。结果33例患者中32例PET/CT显像阳性,肿瘤最常见浸润部位为鼻(21/32)、咽(17/32)。临床最终分期为Ⅰ期14例,Ⅱ期12例,Ⅲ期0例,Ⅳ期6例,PET/CT分期与临床符合率为100.0%(32/32),CT分期与临床符合率为78.1%(25/32)。8例患者因PET/CT结果而调整了治疗方案。SUVmax与上述临床参数间并无联系(P>0.05)。结论18F-FDGPET/CT在结外NK/T细胞淋巴瘤的诊断中有辅助作用,在分期中优于CT,对患者治疗决策的制定有重要价值。
ObjectiveTo summarize the performance of extranodal NK/T-cell lymphoma on 18 F-FDG PET/CT and explore the value of 18 F-FDG PET/CT in its staging and treatment planning.MethodsA retrospective analysis of 33 cases of pathologically confirmed extranodal NK/T-cell lymphoma patients with PET/CT images from October 2012 to September 2018 in the hospital,comparison of PET/CT staging and CT staging results,and to explore the relationship between SUV max and tumor staging,with or without B symptoms,lactic dehydrogenase and Ki-67.ResultsOf the 33 patients,32 patients were positive for PET/CT imaging.The most common invasive sites of the tumor were nasal(21/32)and pharynx(17/32).The final clinical stage was 14 cases in stageⅠ,12 cases in stageⅡ,0 case in stageⅢ,and 6 cases in stageⅣ.The PET/CT staging and clinical coincidence rate was 100.0%(32/32),and the CT staging and clinical coincidence rate was 78.1%(25/32).Eight patients were adjusted for treatment based on PET/CT results.There was no association between SUV max and the above clinical parameters(P>0.05).Conclusion 18 F-FDG PET/CT has a complementary role in the diagnosis of extranodal NK/T-cell lymphoma.It is superior to CT in staging and is of great value in the formulation of treatment decisions.
