摘要
目的探讨食管癌管状胃纵隔化术后胸内吻合口瘘的预防和治疗。方法回顾性分析2013—2018年我院食管癌术中实施管状胃并置入纵隔,术后胸内发生吻合口瘘患者的诊治过程。结果所有15例患者经积极保守治疗,在15d到3个月的时间内瘘腔消失,瘘口闭合,均痊愈。结论食管癌术后胸内吻合口瘘预后差、死亡率高,除充足的术前准备、精练的手术操作外,合理的预防措施、针对的治疗方法能改善吻合口瘘患者的预后。
Objective To investigate the prevention and treatment of intrathoracic anastomotic leakage after tubular gastromediastinalization for esophageal cancer. Methods A retrospective analysis was made on the diagnosis and treatment of patients with anastomotic leakage of thoracic cavity after esophageal cancer operation with tubular stomach and mediastinum insertion in our hospital from 2013 to 2018. Results All 15 patients were cured after cavity conservative treatment. The fistula cavity disappeared and the fistula was closed within 15 days to 3 months. Conclusion The prognosis of intrathoracic anastomotic leakage after esophageal cancer surgery is poor and the mortality rate is high. Besides adequate preoperative preparation and skilled operation, reasonable preventive measures and targeted treatment can improve the prognosis of patients with anastomotic leakage.
